GENERAL TERMS OF USE

EarnWealth Solutions Private Limited (CIN U74900PN2014PTC152269) (‘’EarnWealth’’), a company incorporated under the Companies Act, 1956 of India, having its corporate office at Office No – 409, 4th Floor, C-Wing, KPCT Mall, Wanowari, Pune, Maharashtra 411040 entered an agreement with Indian Health Organisation Private Limited to sell their health care products.

These terms and conditions (“Terms”) between you and Indian Health Organisation Private Limited, a company incorporated under the Companies Act, 1956 of India, having its corporate office at No. 64, Okhla Industrial Estate, Phase-III, New Delhi-110020 (“IHO”) governs your membership with IHO and your use of the membership card issued by IHO (“Preventive Healthcare Plan”), as well as the various health benefit offers extended to you by IHO through its network of voluntarily participating medical professionals / dentists / pharmacies / hospitals, other medical practitioners / vendors who are independent practicing professionals and third party health service providers (“Medical Providers”).

The Terms shall be deemed to have been accepted by you whenever you use IHO’s Preventive Healthcare Plans and/or any of the packages issued by IHO, or avail any of the health benefits arising there from. You are understood to have read and agree to abide by (on your own behalf and on behalf of your dependents/family) the Terms. You are requested not to use the IHO’s Preventive Healthcare Plan or any of the Packages issued by IHO, or avail any of the health benefits arising therefrom, if any of the terms and conditions contained in the Terms is unacceptable to you.

IHO’s PREVENTIVE HEALTHCARE PLAN: The IHO’s Preventive Healthcare Plan’s Membership provides you with access to various specified medical, dental, hospital, diagnostic centres & pharmacies (this service is currently not available in Bhopal, Kochi, Kalyani & Patna) and other treatments / medical services (“Medical Services”) provided by the Medical Providers at a cost which is lower than the fees normally charged by such Medical Provider (“Subsidised Fees”).

USAGE OF IHO’s PREVENTIVE HEALTHCARE MEMBERSHIP PLAN: In order to avail the Medical Services at a Subsidised Rate, you shall be required to present your valid IHO’s Preventive Healthcare Plan Membership Card to the Medical Provider or his/her/its authorised representative prior to receiving any Medical Services and agree to the applicable fee. However, you are advised to contact IHO and state your medical requirements, IHO’s Preventive Healthcare Plan Membership Card details, location and other such details as IHO’s Help desk representative may require, following which IHO shall be able to provide you referral of Medical Providers that match your requirements. You may visit the referred Medical Providers for availing the Medical Services, as per your scheduled appointment’s date & time obtained from the IHO.

Payment to the Medical Provider shall be made directly by you at the time of availing the concerned Medical Services, unless otherwise agreed upon between you and the Medical Provider. In case you wish to confirm the Subsidised Fees offered by a particular Medical Provider, you shall be required to contact the IHO’s helpdesk printed on your IHO’s membership card.      

PRIMARY MEMBERS: The purchaser of an individual IHO’s Preventive Healthcare Plan Membership Card shall automatically be designated as the “Primary Member”.On purchase of a couple / family IHO’s Preventive Healthcare Plan Membership, 1 (one) person from among the couple / family applying for membership is required to be designated at the time of applying as the “Primary Member”, provided the purchaser is above the age of 18 (eighteen) years at the time of purchase.

IHO’s MEMBERSHIP CARD USAGE: Each Membership Card provided by IHO must be used by single person and his/her name must be written with permanent marker on back of each membership card before visiting any medical provider for usage. IHO’s empanelled Medical provider may also ask you to show your ID proof to confirm & match your ID with name printed/written on IHO’s membership card

MEMBERSHIP TERM: On receipt by IHO of your payment of the specified membership fees as well as any enrolment, processing or other fee as specified, you will be issued an IHO’s Preventive Healthcare Membership Card(s) and shall thereafter be entitled to avail the Medical Services at the Subsidised Fees during the membership period as defined by the expiry date given on the face of, or otherwise stated on, your IHO’s Preventive Healthcare Plan Membership Card.

RENEWAL OF MEMBERSHIP: IHO may, at its discretion, inform you of the expiry of your membership at least 1 (one) month in advance. It shall be your responsibility to ensure that IHO receives the specified payment for renewal of membership at least 2 (two) weeks prior to the date of expiry thereof. IHO is not responsible and assumes no liability for any disruption of service in case the payment gets delayed or the payment cheques are dishonoured. In case of dishonoured cheques, a penalty as specified by IHO may be charged to you. The IHO’s Preventive Healthcare Plan’s Membership services will remain suspended till the realization of complete payment towards renewal of membership and all other outstanding payments.

RENEWAL CHARGES: The charges for renewal can be confirmed by calling the IHO’s help desk number 1 (one) month in advance of the date of expiry of your membership. Kindly note that any Medical Services being offered at subsidised rates will not be offered at such rates by the Medical Provider on expiry of your IHO’s Preventive Healthcare Membership Plan. 

GENERAL TERMS AND CONDITIONS:

  • IHO is not a merchant, manufacturer, or provider of the Medical Services or the Additional Health Benefit Services. IHO does not give any warranty, express or implied, as to description, quality, fitness for any particular purpose, productiveness, or any other matter, for any services or merchandise purchased or received by you from a Medical Provider through your Preventive Healthcare Plan Card or any package issued by IHO. In the event that any service received by you is unsatisfactory, you will be required to deal directly with the Medical Provider for any refund or satisfaction of claim. IHO and the Medical Providers reserve the right to revise their charges at any time.

 

  • You, for yourself and on behalf of any family member who uses the services under the Preventive Healthcare Plan Card, hereby forever release, acquit and discharge each of IHO and its employees, officers, directors, agents and affiliates from any and all liabilities, claims, demands, actions and causes of action that you or your legal representative(s) may have by reason of any damage or personal injury sustained as a result of or during the course of the use of any Medical Services and/or any Additional Health Benefit Services. 
  • IHO’s Preventive Healthcare Plans offered by IHO are not health insurance policies. The IHO’s Preventive Healthcare Plans provides subsidised services through the Medical Providers at certain health care centres. The benefits provided through IHO’s Preventive Healthcare Plans are not applicable if you claim insurance benefits on the medical services provided. You are required to pay for all health care services from Medical Providers providing services to members of IHO, unless specifically stated otherwise.

 

  • Medical Providers are independent entities / professionals in private practice and are neither employees nor agents of IHO and/or its parents, subsidiaries or affiliates. All doctors, dentists, laboratories and other medical practitioners/providers are welcome to participate in this social cause and partner with IHO to make a lasting impact on the health of the common man by providing subsidised health care service to IHO members.
  • Certain Medical Services and/or Additional Health Benefit Services, or a portion thereof, are operated / provided over the internet, including through websites owned or operated by Medical Providers. Any interruption or defect in any of part of any Medical Service and/or Additional Health Benefit Service to be provided through such websites due to technical failures of any kind, including any source code malfunction, website hacking or any other related issue, shall not be the liability of IHO or any Medical Provider. Further, such websites may provide links to other internet sites for the convenience of users. Neither IHO nor the relevant Medical Provider is responsible for the availability or content of these external sites, nor do they endorse, warrant or guarantee the products, services or information described or offered at these external sites. It shall be your responsibility to examine the copyright and licensing restrictions of linked pages and to secure all necessary permissions. You are informed to not assume that the external sites will abide by the same provisions as the policies of IHO or the Medical Provider, including either of their privacy policies.

 

  • IHO does not recommend or suggest any particular Medical Provider or promote any particular Medical Provider’s name or practice. You agree that selection of a Medical Provider to avail Medical Services / Additional Health Benefit Services from is your sole discretion and is not based on any representations or recommendations by IHO, and is therefore your sole and exclusive responsibility.
  • The availability of any particular Medical Provider at any time cannot be guaranteed. The list of Medical Providers offering Medical Services at Subsidised Fees and/or the Additional Health Benefit Services is subject to change without notice.

 

  • All medical service providers empanelled with IHO have agreed to participate in this for a social cause. IHO does not have any commercial arrangement with its empanelled medical providers. The Medical Providers have agreed to offer their services at the Subsidised Fees / without any charge, as the case may be, for the cause of increasing health awareness, in particular preventive health care, and in no way shall it be construed that the empanelled Medical Providers are doing so for commercial reasons. Any Medical Provider proved to be offering the Medical Services / Additional Health Benefit Services for commercial reasons may be reported to IHO and, subject to furnishing of adequate proof, such Medical Provider shall be de-empanelled from IHO doctor’s network panel
  • IHO is a facilitator in providing access to quality healthcare services and in helping people save on their health care costs. IHO does not provide any Medical Services or Additional Health Benefit Services and is not responsible for the outcome of any services availed from any Medical Provider. All Medical Services and Additional Health Benefit Services are the responsibility of the concerned Medical Provider, to be carried out in consultation with you.

 

  • IHO offerings are not insurance and do not involve any reimbursement paid to you, or any other person holding a valid IHO’s Preventive Healthcare Plan Membership Card or any other membership card of IHO, for any of the services offered by IHO.
  • IHO reserves the right, in its sole discretion and without any obligation, to make improvements to, or correct any error or omissions in, any portion of the IHO’s Preventive Healthcare Plan Membership Card and/or any package issued to you by IHO and update information from time to time.

 

  • Except as otherwise specifically stated herein, IHO agrees to keep your information confidential as provided in our Privacy Policy. Any personal information, contact details, feedback data or similar information provided by you to IHO shall be treated as confidential information. You expressly acknowledge and agree that the calls made by you to IHO and/or the calls made to you by any Medical Provider shall be recorded for the purposes of quality and for maintaining the record of your health information, and may be shared with a Medical Provider, if required, from time to time.
  • You hereby agree that certain information in relation to the IHO’s Preventive Healthcare Plan any package issued by IHO, including in relation to any of the Medical Services and/or the Additional Health Benefit Services, whether provided by you or generated by IHO / any Medical Provider associated with IHO, may be shared amongst IHO and the Medical Providers strictly for the purposes of carrying out their respective obligations in relation to provision of the Medical Services and/or the Additional Health Benefit Services to you. In case you do not wish for IHO to use your information for such purpose, you shall be required to send a request regarding the same by way of email to service@ihomail.com. However, you expressly acknowledge and understand that doing so may affect the provision of Medical Services and/or the Additional Health Benefit Services to you in such manner as determined by IHO and/or a Medical Provider.

 

  • Subject to the provisions of applicable law, your usage of the IHO’s Preventive Healthcare Plan’s Membership and/or any package issued by IHO entitles IHO to contact you by way of telephone call, SMS or any other mode; provided, however, that if your phone number is registered on DND/DNC registry, communication by way of telephone call and/or SMS shall be in relation to the IHO’s Preventive Healthcare Plan and/or the package used by you and/or your membership with IHO.
  • Neither IHO nor any Medical Provider shall be obliged to provide any services otherwise than in compliance with applicable laws. IHO shall not be responsible for any claims arising out of technical failures which shall include failure in telephone lines, failure of servers, or failure to provide correct updates due to technical snags at any time. Further, IHO shall not be responsible for any claims arising out of any failure(s) which are beyond the control of IHO.

 

  • No Preventive Healthcare Plan Card or any package issued to you by IHO may be redeemed for cash or used to avail of any services / benefits not specified to be part of the IHO’s Preventive Healthcare Plan or the relevant package, as the case may be, at any time.
  • Any package offered by IHO may be issued in such manner and form, whether paper, electronic or otherwise, as it deems fit. If determined by IHO, such packages may be issued to / purchased by, and be redeemable by, members of IHO not holding IHO’s Preventive Healthcare Plan Membership Cards as well.

 

  • Any advice provided by any Medical Provider shall depend solely on the accuracy of the information provided by you. Neither IHO nor any Medical Provider shall be responsible for any damage, physical, psychological or financial, caused to you due to any improper/unintended use of the IHO’s Preventive Healthcare Plan, any package issued to you or any of the benefits available thereunder, which includes but is not limited to the interpretation of the advice given by any Medical Provider by you or by any unqualified personnel.

 

  • You agree to defend, indemnify and hold harmless IHO from and against any and all claims, damages, obligations, losses, liabilities, costs or debt, and expenses (including but not limited to attorney's fees) arising from: (i) any unauthorized or improper access and use of the IHO’s Preventive Healthcare Plan and/or any package issued to you by IHO; (ii) your violation of any term of these Terms; (iii) your violation of any law; or (iv) your violation of the rights of a third party, including the infringement by you of any intellectual property or other right of any person or entity. These obligations will survive any termination of the Terms.
  • You hereby agree that no Medical Provider providing any services over the telephone shall be obligated to provide call-back service in the event of a call-drop or any similar circumstance except as provided in these Terms.

 

  • In no event shall IHO and/or any Medical Provider be liable to you or any third party for any indirect, consequential, exemplary, incidental, special, or punitive damages, including lost profit damages arising from your use of the IHO’s Preventive Healthcare Plan Membership Card and/or any of the services offered to you by IHO in any manner. You shall not be entitled to specific performance of any of the Terms. The foregoing limitation of liability shall apply to the fullest extent permitted by law in the applicable jurisdiction and, regardless of the form of the action, will at all times be limited to the amount paid, if any, by you to IHO for the IHO’s Preventive Healthcare Plan. You specifically agree and understand that neither IHO nor any Medical Provider is liable to you for any content or illegal conduct of any third party and that you alone accept such risk of harm.
  • The views and suggestions expressed by Medical Providers in the course of providing the Medical Services and/or the Additional Health Benefit Services shall in no manner be construed as an endorsement by IHO of any such advice and views and IHO shall not be responsible in any manner for any such advice.

 

  • To the fullest extent permissible by law IHO disclaims all possible warranties or representations  about the accuracy or completeness of the content regarding the Medical Services and/or the Additional Health Benefit Services provided  by Medical Providers, nor can IHO be held liable or responsible for any (i) errors, omissions or inaccuracies of content or submissions; (ii) personal injury or property damage of any nature resulting from your access to and end use of the Medical Services and/or the Additional Health Benefit Services; (iii) any unauthorized access to or use of our secure servers and/or any and all personal information and/or financial information that may be contained therein; (iv) any deletion of data or interruption or discontinuance of transmission to or from or through the Medical Services and/or the Additional Health Benefit Services; and (v) any content or transactions that is contained or may be available outside the Medical Services and/or the Additional Health Benefit Services.
  • IHO shall not be responsible for any deficiency in service on part of any Medical Provider. Any complaint by you regarding the standard of service or quality, or anything in relation to the Medical Services and/or the Additional Health Benefit Services provided by any Medical Provider shall be solely handled and resolved directly by the relevant Medical Provider. IHO shall not be responsible for any such complaints.

 

  • Neither IHO nor any Medical Provider shall assume any liability or responsibility for any claims, damages, consequences and/or orders of any statutory bodies or consumer forums not arising from any default or negligence on the part of IHO or the relevant Medical Provider, as the case may be.
  • IHO is entitled, without any liability whatsoever, to refuse, limit, suspend, vary or discontinue the provision of any of the Medical Services and/or the Additional Health Benefit Services, and any matter relating thereto including but not limited to the manner and the form in which the Medical Services and/or the Additional Health Benefit Services may be made available to you, at any time for any reasonable cause, including but not limited to the following:

 

  • any violation of these Terms or of any applicable law, rule, regulation, order, direction, notification or the like issued by any competent authority;
  • technical failure, modification, up-gradation, variation, relocation, repair and/or maintenance of relevant computer or other systems/equipment;

 

  • any other reason found to be reasonable by IHO at its discretion.
  • Any provision of the terms and conditions that are declared invalid or unenforceable by a court of competent jurisdiction shall be ineffective to the extent of such invalidity or unenforceability without invalidating or rendering unenforceable the remaining provisions hereof.

 

  • IHO reserves the right to modify the Terms from time to time and it shall be your responsibility to keep yourself updated with the Terms. You agree to be bound by such modifications.
  • The provisions of the Terms are personal to you and you may not assign your rights and obligations under the Terms to any person.
  • You agree to comply with the local laws of the territory where you are located for usage of the IHO’s Preventive Healthcare Plan and/or any package issued to you by IHO, and IHO does not have any responsibility for any violation of laws by you. The Terms shall be governed by the laws of India and the courts at New Delhi will have exclusive jurisdiction thereupon.

    CANCELLATION POLICY

    CANCELLATION AND REFUND: In the event that you wish to cancel your membership with IHO and the IHO’s Preventive Healthcare Plan Membership Card issued to you, you shall be required to intimate the same to IHO by calling IHO’s help desk or by sending an email to service@ihomail.com within 14 (fourteen) days of receipt of the IHO’s Preventive Healthcare Plan Membership’s Welcome Pack. You shall further be required to return the IHO’s Preventive Healthcare Plan’s Membership’s Welcome Pack, with all documents and other items provided to you along with the Welcome Pack (“IHO’s Preventive Healthcare Plan Welcome Pack”) by mailing the aforesaid to the IHO Cancellation Department at 64, Okhla Industrial Estate, Phase – III, New Delhi – 110020. 

    On receipt of such intimation and the IHO’s Preventive Healthcare Plan’s Welcome Pack, IHO shall verify the details of your membership and the request for cancellation placed by you. On IHO’s satisfaction that your request is in accordance with these Terms, IHO shall carry out the following (collectively, “Cancellation”):

    • discontinue the IHO’s Preventive Healthcare Plan Membership Card issued to you and your membership with IHO with immediate effect; and

     

    • issue a refund of the amount paid by you as fees towards purchase of such IHO’s Preventive Healthcare Membership Plan  (“Membership Fees”).

    On Cancellation as aforesaid, IHO shall intimate you of the fact of such Cancellation and shall inform you of the method of refund of your Membership Fees. Such Cancellation shall be subject to the following terms:

    • Cancellations shall be considered only if (i) a valid request is made in this regard within 14 (fourteen) days of receipt of the IHO’s Preventive Healthcare Plan’s Welcome Pack by you, and (ii) the entire IHO’s Preventive Healthcare Plan’s Welcome Pack is returned to IHO within 14 (fourteen) days of receipt of the Cancellation request as aforesaid. For calculation of the aforesaid periods, the records of dispatch, delivery and communications available with IHO shall be considered final.

     

    • No Cancellation shall be carried out if you have purchased the IHO’s Preventive Healthcare Membership Plan under any celebratory / festive offer, or any special / discounted offer of any kind whatsoever.
    • No Cancellation shall be carried out if you have purchased the IHO’s Preventive Healthcare Membership Plan under any interest free easy monthly instalment (EMI) option offered by IHO.

     

    • No Cancellation shall be carried out if you have in any way availed any of the services/benefits available under the IHO’s Preventive Healthcare Plan’s Membership prior to confirmation of Cancellation by IHO.
    • In the event a request for Cancellation is received by IHO after dispatch of the IHO’s Preventive Healthcare Plan’s Welcome Pack to you, IHO shall deduct an amount of Rs. 1,000/- (Rupees one thousand only) or 10% (ten per cent) of the Membership Fees, whichever is higher, from your Membership Fees and only the balance shall be refunded to you as part of the Cancellation process.

     

    • In the event that (i) the entire IHO’s Preventive Healthcare Plan’s Welcome Pack is not received by IHO within 14 (fourteen) days of receipt of a valid request for Cancellation by IHO, or (ii) a request for Cancellation is received by IHO after a welcome call has been made by IHO to you wherein you have confirmed your membership, IHO shall be entitled to deduct an amount of Rs. 2,500/- (Rupees two thousand five hundred only) or up to 50% (fifty per cent) of your Membership Fees, whichever is higher, from your Membership Fees and only the balance shall be refunded to you as part of the Cancellation.
    • All Cancellations approved by IHO shall ordinarily be processed within 21 (twenty one) days of receipt by IHO of the IHO’s Preventive Healthcare Plan’s Welcome Pack. Credit of the Membership Fees to your bank account / credit card account upon Cancellation and refund by IHO shall be as per policies and timelines followed by the banks involved and IHO shall not be responsible for any delays in this regard.

     

    LOST CARD & COMPLAINTS

    LOST CARD: In case of loss of the IHO’s Preventive Healthcare Plan Membership Card, a duplicate membership card may be issued by IHO. You shall be required to pay nominal charges of Rs. 500/- (Rupees five hundred only) for issuance of such duplicate card. The tenure of your membership will remain as specified in the initial IHO’s Preventive Healthcare Plan Membership Card.

    COMPLAINTS: Members are advised to address their complaints, if any, to our representatives. They are there to help you out. However, if you are not satisfied, you may write to IHO at our corporate office address or mail us at service@ihomail.com.

    PROVISION OF SERVICES BY MEDICAL PROVIDERS: The provision of Medical Services provided by Medical Providers to you shall, in addition to as otherwise stated, be subject to the following:

    • The Medical Provider will offer the Subsidised Fees on Medical Services offered by the Medical Provider directly within the premises of such IHO’s medical network partner. The Subsidised rate on medical services shall not be applicable to any further service required from any other doctor/hospital, whether referred by the Medical Provider or otherwise. All medical services, tests, medical fields and specializations may not be under the IHO’s Preventive Healthcare Membership Plan. Further, the Subsidised Fees may not be offered on all treatments and procedures offered by the Medical Provider.

     

    • Any existing subsidised service or package offered or planned by a Medical Provider may not be clubbed with the Medical Services being offered at the Subsidised Fees, except at the option of the Medical Provider.
    • Before availing any Medical Service from any Medical Provider, you are advised to clarify in advance all rates and fees, including the Subsidised Fees, and all other details about the treatment, check-up, test, or other services offered by the Medical Provider to your satisfaction from the relevant Medical Provider. IHO will not participate in or be responsible for any dispute related to rates, Subsidised Fees, services offered or any other such detail once any Medical Service has been availed of.

     

    • If a particular Medical Provider does not offer the Subsidised Fees, you may contact IHO, and IHO will inform you of other Medical Providers partnered with IHO. IHO will not be responsible for providing any compensation to you for the charges levied by the Medical Provider in excess of the Subsidised Fees. 

     

     

    SYMPTOM TRACKER T&Cs

    SYMPTOM TRACKER: Specific Medical Providers may provide a recognition and advisory service on the basis of symptoms as specified by the user of such service (“Symptom Tracker”). The Symptom Tracker shall be open to all and shall not require payment of any consideration, unless otherwise specifically stated. The Symptom Tracker is a general, preliminary advisory service only. The Symptom Tracker is not intended to provide specific medical advice, but rather to provide users with information to better understand their health and apparent medical disorders with the appropriate medical advice. No specific medical advice will be provided via the Symptom Tracker, and users are urged to consult with medical experts for diagnosis and for answers to personal medical questions.TheSymptom Tracker is owned and operated by the relevant Medical Provider, and IHO is not in any way responsible for the layout, content or operation of, or the advice provided on the Symptom Tracker.

    ADDITIONAL BENEFITS: In addition to the concessional Medical Services offered, various additional benefits and plans are offered by Medical Providers to members of IHO (“Additional Health Benefit Services”). The features / benefits offered vary depending on the type of membership purchased by you. Certain Additional Health Benefit Services are available only to premier members of IHO, or are required to be purchased separately. Please refer to your Preventive Healthcare Plan Welcome Pack to check what benefits are offered to you. Certain terms and conditions are specific to certain membership plans and are stated below:

    DIAL –A-DOCTOR & 2ND OPINION T&Cs

    • IHO’s Dial – a – Doctor and Second Opinion: If applicable, you are entitled to the following services: (i) provision of interim medical advice over the phone (“IHO’s Dial-a-Doctor Service”); and (ii) provision of medical opinions over the phone regarding serious illnesses (“IHO’s Second Opinion Service”). The Dial-a-Doctor Service and the Second Opinion Service shall, in addition to as stated in these Terms, be subject to the following terms and conditions:

     

    • The Dial-a-Doctor Service and Second Opinion Service shall be provided by qualified doctors and medical practitioners on the rolls of Medical Providers (“Third Party Doctors”).
    • In order to avail of the Dial-a-Doctor Service or the Second Opinion Service, you shall be required to call the number intimated to you by IHO for this purpose. At the time of making such call, you shall be required to state your IHO membership number as provided on your IHO’s Preventive Healthcare Plan Membership Card and the provision of the aforesaid services to you shall be subject to such IHO membership number and your IHO’s Preventive Healthcare Membership Plan being found to be due and valid.

     

    • The Dial-a-Doctor Service and the Second Opinion Service will be available between 10 am - 8 pm on all working days, subject to availability of the concerned doctor. You may need to dial an outstation mobile phone number to access such services. All applicable call charges will be incurred by you.
    • You hereby confirm that you are aware that the Third Party Doctors receiving and attending calls from holders of the IHO’s Preventive Healthcare Plan’s Membership shall have a minimum qualification of Bachelor of Science (BS).

     

    • Any advice provided by any Third Party Doctor shall not be construed as final and shall be subject to a physical check-up conducted by a duly qualified medical professional. In case of any requirement you may have which cannot, in the Third Party Doctor’s reasonable opinion, be addressed over the telephone, you are advised to physically consult the required medical specialist.
    • You hereby acknowledge that the Third Party Doctors providing the Dial-a-Doctor Service shall provide interim medical advice only as such Third Party Doctor cannot check you physically. No sensitive or critical advice or prescription of any kind shall be given to you by any Third Party Doctor providing the Dial-a-Doctor Service.

     

    • You hereby further acknowledge that the Second Opinion Service shall be provided to you on the basis of medical tests undergone by you and opinions taken by you from duly qualified medical professional prior to availing the Second Opinion Service. Any advice provided to you by a Third Party Doctor as part of the Second Opinion Service shall be dependent on such preliminary tests / consultancies.

    DOCTOR CHAT T&Cs

    • IHO’s Chat-with-Doctor Service: If applicable, you are further entitled to the online interim medical advice facility provided Medical Providers to members of IHO (“Chat-with-Doctor Service”). The Chat-with-Doctor Service shall, in addition to as stated in these Terms, be subject to the following terms and conditions:

     

    • The Chat-with-Doctor Service shall also be provided by Third Party Doctors.
    • The Chat-with-Doctor Service may be availed by IHO members as well as non-members in accordance with these Terms. Only general physician consultations will be provided as part of the Chat-with-Doctor Service.

     

    • In order to avail of the Chat-with-Doctor Service, you shall be required to register on the website of IHO (www.indianhealthorganisation.com/login). You shall not be required to pay any amount for such registration. After registration, you may start using the Chat-with-Doctor Service.
    • You shall be entitled to ask general queries by using the Chat-with-Doctor Service, which shall be answered by the Third Party Doctors in real time to the best of their abilities. The Chat-with-Doctor Service is intended to provide you with the following benefits:

     

    •  
      • General advice on your health condition / health issues, probable diagnosis, prognosis etc., as well as on the broad management of your health conditions.
    •  
      • Information on over the counter medication for temporary relief.

     

    •  
      • Doubt clarification on any investigation you have gone through.
    •  
      • Advice on diet for conditions like diabetes, hypertension, weight loss, weight gain etc.

     

    •  
      • Advice on kind of doctors you need to consult for your conditions.
    • The Chat-with-Doctor Service will be available between 8 am - 7 pm on all days (except national holidays), subject to these Terms.

     

    •  In the event you log onto the website of IHO and submit a query at any time other than as specified above, the  service gets converted into a question – answer format whereby the user can send your query in the form of a question on the given e mail ID visible on the chat pop up box , you will get a response within 24 hours via e-mail, else you can log on the next day during live hours
    • For IHO members post 7 pm, the dial a doctor facility is available, where you can have a telephonic chat with the doctors.

     

    • Any advice provided by any Third Party Doctor shall not be construed as final and shall be subject to a physical check-up conducted by a duly qualified medical professional. In case of any requirement you may have which cannot, in the Third Party Doctor’s reasonable opinion, be addressed online, you are advised to physically consult the required medical specialist.
    • You hereby acknowledge that the Third Party Doctors providing the Chat-with-Doctor Service shall provide interim medical advice only as such Third Party Doctor cannot check you physically. No sensitive or critical advice or prescription of any kind shall be given to you by any Third Party Doctor providing the Chat-with-Doctor Service.

     

    • You hereby further acknowledge that you shall be entitled to receive advice on over the counter medications only. The Third Party Doctor shall only prescribe medication which is generally considered to be safe under all circumstances. In case of any allergies, you are advised to undergo personal physical consultation with a nearby doctor.
    Any queries by you in relation to the Chat-with-Doctor Service should be intimated to IHO by sending an email to IHO at doctorchat@ihomail.com, or by calling IHO at 011-60004466 between 9 am and 6 pm (Monday to Saturday). IHO will endeavour to address all queries within 48 (forty eight) working hours.

    IHO- THYROCARE’S T&Cs

     

    1. Thyrocare Health Check Packages: If applicable, you are further entitled to certain packages of diagnostic and investigative tests and services offered by IHO through Thyrocare Technologies Limited (“Thyrocare”) for free / at discounted rates. Such packages may be availed by using either the classic package (“IHO-Thyrocare’s Classic Package”) or the premier package (“IHO- Thyrocare’s Premier Package”) issued by IHO. The Classic IHO-Thyrocare Package entitles you to avail the IHO-Thyrocare Package A and the IHO-Thyrocare Package B, and the IHO-Thyrocare Premier Package entitles you to avail the IHO-Thyrocare Premier Package. “IHO-Thyrocare Package A” means the package of diagnostic and investigative tests and services designated as such, the detailed list of which is located at [http://wellness.thyrocare.com/public_tests_list1.asp]. “IHO-Thyrocare Package B” means the additional package of diagnostic and investigative tests and services designated as such, the detailed list of which is located at [http://wellness.thyrocare.com/public_tests_list1.asp]. “IHO- Thyrocare Premier Package” means the package of diagnostic and investigative tests and services designated as such offered to holders of IHO’s Preventive Healthcare Membership Plans, the detailed list of which is located at [http://www.thyrocare.com/iho.htm]. The aforesaid packages shall, in addition to as stated in these Terms, be subject to the following terms and conditions 
    • The Classic IHO–Thyrocare’s Package and the Premier IHO-Thyrocare’s Package (collectively, the “Thyrocare Packages”) shall be issued by IHO in manner as may be determined by IHO at its sole discretion.

     

    • You may redeem the Classic IHO –Thyrocare’ s Package only once in order to avail the IHO- Thyrocare’s Package A either by itself or along with the IHO –Thyrocare’s Package B. It is hereby clarified that you may avail of IHO-Thyrocare’s Package A as well as IHO- Thyrocare’s Package B only if you avail of both simultaneously. You shall not be entitled to avail the IHO –Thyrocare’s Package’s B without availing the IHO –Thyrocare’s Package A.
    • You may redeem the IHO-Thyrocare’s Premier Package only once in order to avail the IHO   -Thyrocare’s Premier Packages. It is hereby clarified that you may redeem the IHO- Thyrocare’s Premier Package and the IHO- Thyrocare’s Classic Package either separately or simultaneously with each other.

     

    • You may avail the IHO –Thyrocare’s Package A and/or the IHO- Thyrocare’s Premier Package free of any cost, including collection of sample(s) and delivery of reports at the address prescribed by you. You may avail the IHO –Thyrocare’s Package B by paying an amount of Rs. 200 (Rupees two hundred only) directly to Thyrocare technician at the time of collection of your blood sample in case home collection services are availed, or when you visit Thyrocare collection centre.
    • Each Thyrocare Package shall have a unique identification number (“Thyrocare Package Number”) and date of expiry (“Thyrocare Package Expiry Date”) printed/stamped on your package coupon. You may use the relevant Thyrocare Package, subject to the provisions, T & C’s stated on your coupon, at any time up till the Expiry Date.

     

    Usage process for the IHO-Thyrocare’s Health Check Packages

       (a). All IHO –Thyrocare’s (Classic & Premier) package issued till 30th April can be used by  calling Thyrocare to book an appointment to Thyrocare customer care number (022 - 30900000 / 41252525 / 27622762) or by sending an  SMS “IHOP” to 9870666333 or by sending an email to info@thyrocare.com.

    • In case you are unable to communicate with Thyrocare in relation to redemption of a Thyrocare Package through the modes stated herein, you may send an email regarding the same to [service@ihomail.com].

     

    (b). All IHO- Thyrocare’s packages  issued from 1st July 2013 onwards, you shall be required to call IHO Helpdesk for booking of appointment/usage at below numbers:

    (c). At the time of making such booking, you shall be required to state (i) your IHO-Thyrocare’s Package Number and the IHO-Thyrocare’s Package Expiry Date thereof; (ii) if redeeming the Classic IHO-Thyrocare’s  Package, whether you wish to avail of IHO- Thyrocare’s Package A by itself or along with the IHO –Thyrocare’s Package B; (iii) address for collection of sample(s) and delivery of report; (iv) your contact number, preferably your mobile phone number; and (v) 2 (two) alternative convenient dates and timings for collection of sample(s) by authorised technician sent by Thyrocare. The details of the booking placed with IHO/ Thyrocare shall be sent to you by way of an email and an SMS. You may avail the IHO –Thyrocare’s Premier Package, the IHO-Thyrocare’s Package A and/or the IHO- Thyrocare’s Package B by handing over a copy of your valid & booked IHO-Thyrocare‘s Package.

     

    • You shall be required to undergo fasting for a minimum of 12 (twelve) hours prior to providing sample(s) for any of the tests specified. It is recommended that you undergo overnight fasting prior to provision of sample(s). Additional directions may be provided to you by Thyrocare at the time of placing the booking, if required. IHO shall not be responsible in any manner whatsoever if you do not undergo fasting as prescribed herein or follow any directions provided to you.
    • You shall be required to obtain confirmation directly from Thyrocare regarding availability of authorised Thyrocare service centre and/or provision for collection of samples and delivery of reports at address prescribed by you (“Home Service”) in your city. In the event such Home Service is not available in your city, you shall be required to visit the nearest Thyrocare service centre for providing sample(s) and collecting reports. The list of cities where Thyrocare service centre is located is subject to change at any time without prior notice.

     

    • Reports for the tests carried out shall be made available within 3 (three) to 5 (five) days after collection of sample(s) at (i) the address specified by you, if Home Service is available at your location; or (ii) the Thyrocare service centre where you provided the sample(s). Reports may also be provided to you by email if requested.
    • In the event of cancellation of tests by Thyrocare for various clinical reasons such as the sample becoming unfit for processing due to inadvertent reasons like hemolysation or clotting or quality deterioration of sample, sample becoming insufficient due to multiple retesting of certain parameters, absurd values generated due to reasons not under control etc., Thyrocare may re-conduct the test/investigation desired by you. Such retesting, including collection of sample(s) from and delivery of reports to the address specified by you, shall be conducted free of any costs or charges of any nature whatsoever to you. However if the reports for the test/investigation desired by you are released and you thereafter ask for retesting, you shall be charged for such retesting as per the prevailing Thyrocare rates.

     

    • Neither IHO nor Thyrocare shall be responsible for any delays in getting the sample(s) to the central processing laboratory of Thyrocare or in delivery of reports which may arise due to causes beyond their organisational control such as delay by courier agency, unexpected cancellation of commercial flights due to extreme weather conditions, natural calamities or any other force majeure condition, or your absence or unavailability at the address specified by you when report delivery is attempted. In such an event, Thyrocare may, at its discretion and on a case to case basis, re-conduct the test/investigation desired by you, including collection of sample(s) from and delivery of reports to the address specified by the you, free of any costs or charges of any nature whatsoever to you.
    • The IHO-Thyrocare’s Premier Package, the IHO-Thyrocare’s Package A and/or the IHO –Thyrocare’s Package B may be withdrawn at any time including during the validity period of the relevant Thyrocare’s Package at the sole discretion of IHO with or without prior notice.

     

    • Any results generated from any tests conducted through usage of any Thyrocare Package may not be used for any medico-legal purposes.
    • All members test reports will be shared by Thyrocare with IHO and IHO may use this information, in case an IHO member would not like IHO to use your test reports, he/she must send the mail request (at least 3 days before tests to service@ihomail.com)

     

    • Thyrocare shall be solely responsible for any defect or deficiency with regard to conducting tests/investigations, collection of sample(s) and preparation and delivery of reports, including without limitation deficiency in quality of testing, default in meeting timelines or otherwise. IHO shall not be responsible for any deficiency in service on part of Thyrocare. Any complaint by you regarding the standards of service or quality, or anything in relation to the services provided by Thyrocare, shall be solely handled and resolved directly by Thyrocare. IHO shall not be responsible for any such complaint.
    • Neither IHO nor Thyrocare shall be responsible for any issues arising in case you directly avail service from a service provider without making a prior booking with Thyrocare as per the procedure stated herein.

     

    • All IHO-Thyrocare’s packages are valid till the IHO membership is valid.

    DIET FIT PACKAGE T&Cs

    • Diet Fit Package: If applicable, you are further entitled to customized diet packages designed and provided by Medical Providers to members of IHO (“Diet Fit Packages”). Such diet packages may be availed by using the specialised package issued by IHO for this purpose (“Diet Fit Package”). The Diet Fit Packages shall, in addition to as stated in these Terms, be subject to the following terms and conditions: 

     

    • The Diet Fit Package shall be issued by IHO to you in such manner as may be determined by IHO at its sole discretion. The IHO’s Diet Fit Packages may be redeemed only by holders of a valid IHO’s Diet Fit Package. The IHO’s Diet Fit Package shall have a unique identification number (“IHO’s Diet Fit Package Number”) and date of expiry (“IHO’s Diet FitPackage Expiry Date”) provided on it. You may use the IHO’s Diet Fit Package to avail of the relevant IHO’s Diet Fit Package, subject to the provisions stated in these Terms, at any time up till the IHO’s Diet Fit Package Expiry Date.
    • You shall be entitled to receive the following as part of every IHO’s Diet Fit Package:

     

    •  
      • Phone call from an experienced specialist, including a duly qualified doctor and nutritionist, on the roster of a Medical Provider (“Dietician”) to understand your overall lifestyle pattern and to provide customised diet plans to you based on the information provided by you (“IHO’s Diet Fit Consultation Call”);
    •  
      • Phone call from the Dietician enquiring about how and what has been your development so far (“Follow-up Call”);

     

    •  
      • Daily SMS consisting of dietary health tips customised as per your medical health requirements (“Diet Fit SMS”); and
    •  
      • Email newsletter consisting of diet plans, recipes and diet charts customised as per your requirements along with general dietary health tips (“Diet Fit Email”).

     

    • The IHO’s Diet Fit Package purchased by you may be of a duration of (a) 1 (one) month (“One Month Diet Fit Package”); (b) 3 (three) month (“Three Month Diet Fit Package”); or (c) 6 (six) months (“ Six Month Diet Fit Package”). You shall be entitled to the following based on the Diet Fit Package purchased by you:
    • The One Month’s IHO’s Diet Fit Package shall consist of:
    • 1 (one) Diet Fit Consultation Call;
  • 1 (one) Follow-up Call;
  • 30 (thirty) Diet Fit SMSs; and
  • 4 (four) Diet Fit Emails.
  • The Three Months IHO’s Diet Fit Package shall consist of:
  • 1 (one) Diet Fit Consultation Calls;
  • 2 (two) Follow-up Calls;
    • 90 (ninety) Diet Fit SMSs; and
    • 12 (twelve) Diet Fit Emails.
    • The Six Months IHO’s Diet Fit Package shall consist of:
    • 1 (one) Diet Fit Consultation Calls;
    • 5 (five) Follow-up Calls;
    • 180 (one hundred and eighty) Diet Fit SMSs; and
    • 24 (twenty four) Diet Fit Emails.
    • To avail of the relevant IHO’s Diet Fit Package, you shall be required to contact IHO by calling the relevant help-desk number below between 9 am and 6 pm (Monday to Saturday) (“Diet Fit Registration Call”):

    USAGE PROCESS / VALIDITY:

    • You shall be required to make the IHO’s Diet Fit Registration Call as aforesaid within the following periods:

     

    •  
      • If you have purchased the One Month IHO’s Diet Fit Package, within 45 (forty five) days of the date of purchase of the IHO’s Diet Fit Package;
    •  
      • If you have purchased the Three Months IHO’s Diet Fit Package, within 90 (ninety) days of the date of purchase of the IHO’s Diet Fit Package; and

     

    •  
      • If you have purchased the Six Months Diet Fit Package, within 90 (ninety ) days of the date of purchase of the IHO’s Diet Fit Package.
    • At the time of making the IHO’s Diet Fit Package’s Registration Call to the IHO helpdesk, you shall be required to state your IHO’s Diet Fit Package Number as provided on your IHO’s Diet Fit Package and the provision of benefits under the IHO’s Diet Fit Packages to you shall be subject to such IHO’s Diet Fit Package Number being found to be due and valid. You shall also be required to provide certain additional details to the IHO help desk executive during the IHO’s Diet Fit Registration Call, including the following:

     

    •  
      • Full demographic details as requested;
    •  
      • Contact details (valid mobile number and email ID);

     

    •  
      • Vital statistics, i.e., height in feet, weight in kilograms, waist circumferences in centimetres, etc;
    •  
      • IHO’s Diet Fit Package’s duration;

     

    •  
      • Purchase date of the relevant IHO’s Diet Fit Package;
    •  
      • IHO’s Diet Fit Package’s Expiry Date.

     

    • In addition to the above, the IHO help desk representative shall fill out a questionnaire about your lifestyle pattern and dietary habits (“Questionnaire”) on your behalf during the IHO’s Diet Fit Registration Call. For such purpose, you shall be required to provide all information as may be requested by the IHO help desk representative.
    • You shall further be required to provide to the IHO help desk representative during the IHO’s Diet Fit Registration Call, to be included in the Questionnaire, two alternative convenient dates and times on which the Dietician may make the Consultation Call. Such convenient dates and times should be not less than 48 (forty eight) hours and not more than 7 (seven) days from the time of the Registration Call.

     

    • Within 24 (twenty four) hours of successful completion of the IHO’s Diet Fit Package’s Registration Call, you shall be sent an e-mail and/or an SMS confirming submission of the Questionnaire and stating the date and time, from amongst the two alternative dates and times specified by you, on which the Consultation Call shall occur. The email and/or SMS sent to you shall also contain a link to a website along with login details and password which you may use to access your customised IHO’s Diet Fit Package portal page. You shall be required to access such customised IHO’s Diet Fit Package portal page using the login details and password provided within 7 (seven) days of receiving the same, post which the link and login details as provided shall be deactivated.
    • You may modify the preferred dates and times for the Consultation Call provided by you during the IHO’s Diet Fit Package’s Registration Call through your IHO’s Diet Fit Package’s portal page at any time prior to 24 (twenty four) hours before your scheduled Consultation Call, provided that such rescheduled date and time should not be less than 48 (forty eight) hours from the time of submission of your amended date and time.

     

    • On the date and time stated, the Dietician shall make the Consultation Call. Such Consultation Call shall not be made unless the Questionnaire is duly filled up and submitted as required. The Consultation Call may last up to approximately 30 (thirty) minutes and you should make himself available accordingly. In the event you are not available at the specified time, the Dietician shall call later on the same date or, failing that, on the immediately succeeding day.
    • Provision of the IHO’s Diet Fit Package SMSs to you shall commence within 24 (twenty four) hours of the Consultation Call. Further, provision of the IHO’s Diet Fit Package Emails to you shall commence within 48 (forty eight) hours of the Consultation Call.

     

    • You shall receive the first Follow-up Call within 15 (fifteen) days of the Consultation Call. Any further Follow-up Calls to be made based on the Diet Fit Package purchased by you shall be made not later than 30 (thirty) days after the immediately preceding Follow-up Call. In the event the Dietician is unable to reach you for any Follow-up Call, he/she shall make the relevant Follow-up Call later during the same day or on the immediately succeeding day.
    • The IHO’s Diet Fit Package SMSs shall be sent to you once a day during the tenure of your Diet Fit Package on the mobile number provided by you to IHO help desk representative during the Registration Call. Further, the IHO’s Diet Fit Package Emails shall be sent to you once a week during the tenure of your IHO’s Diet Fit Package on the email address provided by you to IHO. The number of IHO’s Diet Fit Package SMSs and IHO’s Diet Fit Package Emails shall however be subject to the maximum prescribed hereinabove for the relevant IHO’s Diet Fit Package purchased by you.

     

    • In the event you wish to extend the tenure of your IHO’s Diet Fit Package, you shall be required to purchase a new IHO’s Diet Fit Package by contacting IHO by calling the relevant help-desk number provided hereinabove between 9 am and 6 pm (Monday to Saturday) (“Diet Fit Renewal Call”). During the IHO’s Diet Fit Package’s Renewal Call, you shall be required to make payment for the relevant new IHO’s Diet Fit Package you wish to purchase. Further, at the time of making the IHO’s Diet Fit Package’s Renewal Call, you shall be required to provide certain details to the IHO help desk representative as requested during the IHO’s Diet Fit Package’s Renewal Call, as well as provide all requisite details to the IHO help desk representative in order to enable the IHO help desk representative to complete a short questionnaire (“Package Renewal Questionnaire”).
    • All provisions of these Terms in relation to the IHO’s Diet Fit Package’s Registration Call shall apply mutatis mutandis to the Diet Fit Package’s Renewal Call, and all provisions of these Terms in relation to the Questionnaire shall apply mutatis mutandis to the Renewal Questionnaire. Further, the procedure as described in these Terms after successful completion of the IHO’s Diet Fit Registration Call shall be mutatis mutandis applicable after successful completion of the IHO’s Diet Fit Package’s Renewal Call.

     

    • The truncated procedure relating to IHO’s Diet Fit Package’s Renewal Call and Renewal Questionnaire as specified hereinabove shall be applicable only if (a) the Diet Fit Renewal Call is made by you to IHO by calling the relevant help-desk number as provided above within 1 (one) year of expiry of your previous Diet Fit Package; and (b) the user of the initial IHO’s Diet Fit Package remains the user of the subsequently purchased Diet Fit Package. In case you wish to purchase a new IHO’s Diet Fit Package after the expiry of the aforesaid period of 1 (one) year, or if the user of the new IHO’s Diet Fit Package has changed, you shall be required to purchase an IHO’s Diet Fit Package and comply with the provisions relating to IHO’s Diet Fit Package’s Registration Call and Questionnaire as specified hereinabove.
    • You hereby acknowledge and agree that you shall not be entitled to any of the benefits arising from the relevant IHO’s Diet Fit Package purchased by you in the event any of the actions required to be taken by you as stated in these Terms are not duly taken by you within the relevant time periods specified herein.

     

    • You hereby acknowledge that the Dieticians shall provide interim medical advice only as such Dietician cannot check you physically. Any advice provided by any Dietician shall not be construed as final and shall be subject to a physical check-up conducted by a duly qualified medical professional. No sensitive or critical advice or prescription of any kind shall be given to you by any Dietician. In case of any requirement you may have which cannot, in the Dietician’s reasonable opinion, be addressed over the telephone, you are advised to physically consult the required medical specialist.
    • Any queries by you in relation to the IHO’s Diet Fit Package should be intimated to IHO by sending an email to IHO at diet.q@ihomail.com or by calling the relevant help-desk number provided hereinabove between 9 am and 6 pm (Monday to Saturday). IHO will endeavour to address all queries within 48 (forty eight) working hours.

     

    PRE-NATAL CARE PACKAGE’S T&Cs

    • Pre-natal Care Package If applicable, you are further entitled to pre-natal care related health information provided by Medical Providers to members of IHO (“Pre-natal Care Package”). Such programs may be availed by using the specialised package issued for this purpose (“Pre-natal Care Program Package”). The Pre-natal Care Package shall, in addition to as stated in these Terms, be subject to the following terms and conditions: 

     

    • The Pre-natal Care Program Package shall be issued by IHO to you in such manner as may be determined by IHO at its sole discretion. Pre-natal Care Programs may be availed of only by holders of a valid Pregnancy Plan Package. The Pre-natal Care Program Package shall have a unique identification number (“Pre-natal Care ProgramNumber”) and date of expiry (“Pre-natal Care Package’s Expiry Date”) provided on it. In the event you hold a membership with IHO, the Pre-natal Care Package’s Expiry Date shall be the earlier of (i) the date of expiry printed on the Pre-natal Care Package, or (ii) the last date of validity of your IHO membership. You may use the Pre-natal Care Package to avail of the Pre-natal Care Package, subject to the provisions stated in these Terms, at any time up till the Pre-natal Care Package’s Expiry Date.

    • You shall be required to complete the Pre-natal Care Package’s Registration as aforesaid, prior to the Pre-natal Care Package’s Expiry Date, between the date of conception and the pregnancy due date.

     

    • At the time of Pre-natal Care Package’s Registration, you shall be required to state your Pre-natal Care Package Number as provided on your Pre-natal Care Package and the provision of benefits under the Pre-natal Care Package to you shall be subject to such Pre-natal Care Package Number being found to be due and valid. You shall also be required to provide certain additional details as part of the Pre-natal Care Package’s Registration, including the following:
    •  
      • Full demographic details as requested;

     

    •  
      • Contact details (valid mobile number and email ID);
    •  
      • Delivery due date;

     

    •  
      • First day of last menstrual period;
    •  
      • Number of days of pregnancy;

     

    •  
      • Pre-natal Care Package’s Expiry Date.
    • Within 24 (twenty four) hours of successful completion of the Pre-natal Care Package’s Registration, you shall be sent an e-mail and/or an SMS containing your login details and password for accessing your customised portal (“Customer Portal”) along with link for your customised portal. You may visit your Customer Portal using the login details and password provided at any time during the validity period of your Pre-natal Care Package.

     

    • Upon completion of the Pre-natal Care Package’s Registration, you shall be entitled to receive the following:
    •  
      • Phone consultations with various medical specialists on the roster of a Medical Provider (“Specialists”) in accordance with your needs (“Pre-natal Care Package’s ConsultationCall”);

     

    •  
      • SMS consisting of estimated changes occurred / occurring in the baby based on estimated stage of pregnancy calendar (“Pre-natal Care Package SMS”);
    •  
      • Email consisting of estimated changes occurred / occurring in the mother and the baby (“Pre-natal Care Package Email”); and

     

    •  
      • Email newsletter aimed at education and information, containing tips on nutrition, pre-natal care, yoga and relaxation techniques, balancing work pressure and pre-natal care, diet, emotional health etc. (“Pre-natal Care Package Newsletter”).
    • All benefits provided to you as part of the Pre-natal Care Package shall be provided to you commencing from the successful completion of the Pre-natal Care Package’s Registration till the earlier of (i) your delivery due date as specified by you; or (ii) the expiry of a period of 9 (nine) months from the Pre-natal Care Package’s Registration.

     

    • Provision of Pre-natal Care Package SMSs to you shall commence within 24 (twenty four) hours of the Pre-natal Care Package’s Registration. Further, provision of Pre-natal Care Program Emails and Pre-natal Care Package Newsletters to you shall commence within 48 (forty eight) hours of the Pre-natal Care Package’s Registration.
    • The Pre-natal Care Package’s SMSs shall be sent to you once a day during the tenure of your Pre-natal Care Package. The Pre-natal Care Package’s Emails shall be sent to you once a week during the tenure of your Pre-natal Care Package. The Pre-natal Care Package Newsletters shall be sent to you once a month during the tenure of your Pre-natal Care Package. All of the aforesaid shall be sent on the mobile phone number or email, as the case may be, provided by you during the Pre-natal Care Package’s Registration.

     

    • You shall be entitled to have Pre-natal Care Package Consultation Calls with the following Specialists: (i) general physician, (ii) dietician, (iii) gynaecologist, or (iv) psychologist.
    • In order to have a Pre-natal Care Package Consultation Call, you shall be required to call IHO on 011-60004466 between 9 am and 6 pm (Monday to Saturday). In case you wish to consult with a general physician or a dietician, you may either (i) book a future date and time within the operating hours as specified below for such call, or (ii) have the Pre-natal Care Package Consultation Call immediately if within the operating hours specified below, in which case the IHO help-desk representative shall connect your call to the relevant Specialist. In case you wish to consult with a gynaecologist or a psychologist, you shall be required to book a future date and time within operating hours to have the Pre-natal Care Package Consultation Call. Each time you wish to have / book a Pre-natal Care Package Consultation Call, you may choose a particular Specialist to consult by checking the credentials of each Specialist on your Customer Portal.

     

    • Any Pre-natal Care Package Consultation Call booked by you shall be required to be within the operating hours of 9 am and 6 pm (Monday to Friday) and shall be not less than 2 (hours) after the booking call made by you.
    • You may modify the date and time of the booked Pre-natal Care Package Consultation Cal by calling the relevant IHO help-desk number as stated hereinabove, provided that such rescheduled date and time should not be less than 2 (two) hours from the time of amended booking made by you.

     

    • On the date and time stated, the relevant Specialist shall make the Pre-natal Care Program Consultation Call by calling you on the phone number proved by you during Pre-natal Care Program Registration. The Pre-natal Care Package Consultation Call should be specifically related to your pregnancy and you should make yourself available accordingly. In the event you are not available at the specified time or within a reasonable amount of time afterwards, the Specialist shall not be responsible for calling you again, and you shall be required to re-book a Pre-natal Care Package’s Consultation Call.
    • Even though this package is valid for 9 months, this package will expire at the time of delivery irrespective of date of registration or usage days of this package. IHO’s network concession benefits under this package will only be available till the validity of your IHO’s membership card.

     

    • After successful completion of any Pre-natal Care Package’s Consultation Call, you shall receive an email from the relevant Specialist containing feedback from such Pre-natal Care Package’s Consultation Call.
    • You shall also be entitled to use your Customer Portal for raising queries related to your pregnancy, which shall be answered by the relevant Specialists to the best of their abilities. In addition, you may also upload your pre-natal and related medical records on your Customer Portal for storage as well as review by the Specialists. Further, in the event you undergo testing at any diagnostic centre associated with IHO wherein such service is provided, you shall have the option of requiring such diagnostic centre to directly upload your test results on your Customer Portal. All records uploaded to your Customer Portal shall be maintained and shall be available till the validity of your Pre-natal Care Package, i.e., till the earlier of (i) your delivery due date as specified by you; or (ii) the expiry of a period of 9 (nine) months from the Pre-natal Care Package’s Registration. You shall not be able to access any of your records after the validity as aforesaid is over. Further, all such uploaded records shall be visible to all specialists as well as to IHO, and you agree to allow IHO and the specialists to view such records at any time during the validity of your Pre-natal Care Package.

     

    • You hereby acknowledge and agree that you shall not be entitled to any of the benefits arising from the relevant Pre-natal Care Package purchased by you in the event any of the actions required to be taken by you as stated in these Terms are not duly taken by you within the relevant time periods specified herein.
    • You hereby acknowledge that any information provided to you through any Pre-natal Care Package’s SMS, Pre-natal Care Package’s Email, and Pre-natal Care Package’s Newsletter or otherwise shall be based on the information provided by you as well as certain estimates made by the Specialists on the basis of the information provided by you, including estimates of your pregnancy cycle. As such, all such information shall be tentative and shall be subject to verification by way of physical check-up conducted by a duly qualified medical professional.

     

    • In the event you hold an IHO membership, you shall also be entitled to all benefits available to you under your IHO membership, including benefits on gynaecology consultations, laboratory investigations (pathology and radiology) and delivery concession at IHO empanelled medical centres and hospitals. For further details on network benefits, you may contact IHO help-desk at 011-60004466.
    • You hereby acknowledge that the Specialists shall provide interim medical advice only as such Specialists cannot check you physically. Any advice provided by any Specialist shall not be construed as final and shall be subject to a physical check-up conducted by a duly qualified medical professional. No sensitive or critical advice or prescription of any kind shall be given to you by any Specialist. In case of any requirement you may have which cannot, in the Specialist’s reasonable opinion, be addressed over the telephone, you are advised to physically consult the required medical professional.

     

    • Any queries by you in relation to the Pre-natal Care’s Package or the Pre-natal Care Package should be intimated to IHO by sending an email to IHO at maternity.q@ihomail.com, or by calling the relevant help-desk number provided hereinabove between 9 am and 6 pm (Monday to Saturday). IHO will endeavour to address all queries within 48 (forty eight) working hours.

    DENTAL PACKAGES T&Cs

    • Dental Package: If applicable, you are further entitled to certain dental scaling and cleaning services provided by dental service providers to members of IHO (“Dental Package”). Such package may be availed by using the specialised package issued by IHO for this purpose (“Dental Package”). The Dental Package shall, in addition to as stated in these Terms, be subject to the following terms and conditions: 

     

    • The Dental Package shall be issued by IHO to you in such manner as may be determined by IHO at its sole discretion. The Dental Package may be redeemed only by holders of a valid Dental Package. The Dental Package shall have a unique identification number (“Dental Package Number”) and date of expiry (“Dental Package Expiry Date”) provided on it. You may use the Dental Package to avail of the Dental Package, subject to the provisions stated in these Terms, at any time up till the Dental Package Expiry Date.
    • As part of the Dental Package, you shall be entitled to one session of basic oral prophylaxis treatment consisting of ultrasound cleaning and scaling of teeth free of cost. The Dental Package does not include any advanced cleaning processes like ozone based cleaning or any other process or benefit whatsoever.

     

    • To use the Dental Package, you shall be required to contact IHO by calling IHO at 011-60004466 between 9 am and 6 pm (Monday to Saturday) (“Booking Call”):
    • At the time of making the Booking Call, you shall be required to state your Dental Package Number and Dental Package Expiry Date thereof, and the provision of benefits under the Dental Package to you shall be subject to such Dental Package being found to be due and valid.

     

    • On receiving a Booking Call, the representative of IHO shall inform you of the Medical Providers with whom the Dental Package may be availed situated nearest to your location as intimated to IHO. The final selection of any of the alternatives provided shall be at your sole discretion.
    • You shall be required to provide 2 (two) alternative dates and times for making a booking for availing the benefits under the Dental Package to the IHO representative during the Booking Call. In the event a booking is unable to be made during either of the 2 (two) alternative dates and times provided by you, a representative of IHO shall call you on the number provided by you during the Booking Call with suggested alternative dates and times for such booking. You shall be required to confirm the booking at such date and time.

     

    • When a booking is successfully made for either of the alternatives as specified by you, or for the suggested date and time confirmed by you subsequently, IHO shall send confirmation of such booking to you by way of SMS to the number provided by you during the Booking Call.
    • You shall be required to present a copy of your Dental Package at the time of availing the Dental Package.
    • In the event you wish to change your booking date and time, you shall be entitled to do so by calling the relevant help-desk number during the times mentioned hereinabove not less than 24 (twenty four) hours prior to your scheduled booking. Any re-booking shall be subject to availability of the Medical Provider. In the event you fail to call the relevant IHO help-desk number within the time period as aforesaid, you shall not be entitled to any re-booking or further opportunity to avail the benefits of the Dental Package.

    These terms and conditions (“Terms”) between you and Indian Health Organisation Private Limited, a company incorporated under the Companies Act, 1956 of India, having its corporate office at No. 64, Okhla Industrial Estate, Phase-III, New Delhi-110020 (“IHO”) governs your membership with IHO and your use of the membership card issued by IHO (“Preventive Healthcare Plan”), as well as the various health benefit offers extended to you by IHO through its network of voluntarily participating medical professionals / dentists / pharmacies / hospitals, other medical practitioners / vendors who are independent practicing professionals and third party health service providers (“Medical Providers”).

    The Terms shall be deemed to have been accepted by you whenever you use IHO’s Preventive Healthcare Plans and/or any of the packages issued by IHO, or avail any of the health benefits arising there from. You are understood to have read and agree to abide by (on your own behalf and on behalf of your dependents/family) the Terms. You are requested not to use the IHO’s Preventive Healthcare Plan or any of the Packages issued by IHO, or avail any of the health benefits arising therefrom, if any of the terms and conditions contained in the Terms is unacceptable to you.

    IHO’s PREVENTIVE HEALTHCARE PLAN: The IHO’s Preventive Healthcare Plan’s Membership provides you with access to various specified medical, dental, hospital, diagnostic centres & pharmacies (this service is currently not available in Bhopal, Kochi, Kalyani & Patna) and other treatments / medical services (“Medical Services”) provided by the Medical Providers at a cost which is lower than the fees normally charged by such Medical Provider (“Subsidised Fees”).

    USAGE OF IHO’s PREVENTIVE HEALTHCARE MEMBERSHIP PLAN: In order to avail the Medical Services at a Subsidised Rate, you shall be required to present your valid IHO’s Preventive Healthcare Plan Membership Card to the Medical Provider or his/her/its authorised representative prior to receiving any Medical Services and agree to the applicable fee. However, you are advised to contact IHO and state your medical requirements, IHO’s Preventive Healthcare Plan Membership Card details, location and other such details as IHO’s Help desk representative may require, following which IHO shall be able to provide you referral of Medical Providers that match your requirements. You may visit the referred Medical Providers for availing the Medical Services, as per your scheduled appointment’s date & time obtained from the IHO.

    Payment to the Medical Provider shall be made directly by you at the time of availing the concerned Medical Services, unless otherwise agreed upon between you and the Medical Provider. In case you wish to confirm the Subsidised Fees offered by a particular Medical Provider, you shall be required to contact the IHO’s helpdesk printed on your IHO’s membership card.      

    PRIMARY MEMBERS: The purchaser of an individual IHO’s Preventive Healthcare Plan Membership Card shall automatically be designated as the “Primary Member”.On purchase of a couple / family IHO’s Preventive Healthcare Plan Membership, 1 (one) person from among the couple / family applying for membership is required to be designated at the time of applying as the “Primary Member”, provided the purchaser is above the age of 18 (eighteen) years at the time of purchase.

    IHO’s MEMBERSHIP CARD USAGE: Each Membership Card provided by IHO must be used by single person and his/her name must be written with permanent marker on back of each membership card before visiting any medical provider for usage. IHO’s empanelled Medical provider may also ask you to show your ID proof to confirm & match your ID with name printed/written on IHO’s membership card

    MEMBERSHIP TERM: On receipt by IHO of your payment of the specified membership fees as well as any enrolment, processing or other fee as specified, you will be issued an IHO’s Preventive Healthcare Membership Card(s) and shall thereafter be entitled to avail the Medical Services at the Subsidised Fees during the membership period as defined by the expiry date given on the face of, or otherwise stated on, your IHO’s Preventive Healthcare Plan Membership Card.

    RENEWAL OF MEMBERSHIP: IHO may, at its discretion, inform you of the expiry of your membership at least 1 (one) month in advance. It shall be your responsibility to ensure that IHO receives the specified payment for renewal of membership at least 2 (two) weeks prior to the date of expiry thereof. IHO is not responsible and assumes no liability for any disruption of service in case the payment gets delayed or the payment cheques are dishonoured. In case of dishonoured cheques, a penalty as specified by IHO may be charged to you. The IHO’s Preventive Healthcare Plan’s Membership services will remain suspended till the realization of complete payment towards renewal of membership and all other outstanding payments.

    RENEWAL CHARGES: The charges for renewal can be confirmed by calling the IHO’s help desk number 1 (one) month in advance of the date of expiry of your membership. Kindly note that any Medical Services being offered at subsidised rates will not be offered at such rates by the Medical Provider on expiry of your IHO’s Preventive Healthcare Membership Plan. 

    GENERAL TERMS AND CONDITIONS:

    • IHO is not a merchant, manufacturer, or provider of the Medical Services or the Additional Health Benefit Services. IHO does not give any warranty, express or implied, as to description, quality, fitness for any particular purpose, productiveness, or any other matter, for any services or merchandise purchased or received by you from a Medical Provider through your Preventive Healthcare Plan Card or any package issued by IHO. In the event that any service received by you is unsatisfactory, you will be required to deal directly with the Medical Provider for any refund or satisfaction of claim. IHO and the Medical Providers reserve the right to revise their charges at any time.

     

    • You, for yourself and on behalf of any family member who uses the services under the Preventive Healthcare Plan Card, hereby forever release, acquit and discharge each of IHO and its employees, officers, directors, agents and affiliates from any and all liabilities, claims, demands, actions and causes of action that you or your legal representative(s) may have by reason of any damage or personal injury sustained as a result of or during the course of the use of any Medical Services and/or any Additional Health Benefit Services. 
    • IHO’s Preventive Healthcare Plans offered by IHO are not health insurance policies. The IHO’s Preventive Healthcare Plans provides subsidised services through the Medical Providers at certain health care centres. The benefits provided through IHO’s Preventive Healthcare Plans are not applicable if you claim insurance benefits on the medical services provided. You are required to pay for all health care services from Medical Providers providing services to members of IHO, unless specifically stated otherwise.

     

    • Medical Providers are independent entities / professionals in private practice and are neither employees nor agents of IHO and/or its parents, subsidiaries or affiliates. All doctors, dentists, laboratories and other medical practitioners/providers are welcome to participate in this social cause and partner with IHO to make a lasting impact on the health of the common man by providing subsidised health care service to IHO members.
    • Certain Medical Services and/or Additional Health Benefit Services, or a portion thereof, are operated / provided over the internet, including through websites owned or operated by Medical Providers. Any interruption or defect in any of part of any Medical Service and/or Additional Health Benefit Service to be provided through such websites due to technical failures of any kind, including any source code malfunction, website hacking or any other related issue, shall not be the liability of IHO or any Medical Provider. Further, such websites may provide links to other internet sites for the convenience of users. Neither IHO nor the relevant Medical Provider is responsible for the availability or content of these external sites, nor do they endorse, warrant or guarantee the products, services or information described or offered at these external sites. It shall be your responsibility to examine the copyright and licensing restrictions of linked pages and to secure all necessary permissions. You are informed to not assume that the external sites will abide by the same provisions as the policies of IHO or the Medical Provider, including either of their privacy policies.

     

    • IHO does not recommend or suggest any particular Medical Provider or promote any particular Medical Provider’s name or practice. You agree that selection of a Medical Provider to avail Medical Services / Additional Health Benefit Services from is your sole discretion and is not based on any representations or recommendations by IHO, and is therefore your sole and exclusive responsibility.
    • The availability of any particular Medical Provider at any time cannot be guaranteed. The list of Medical Providers offering Medical Services at Subsidised Fees and/or the Additional Health Benefit Services is subject to change without notice.

     

    • All medical service providers empanelled with IHO have agreed to participate in this for a social cause. IHO does not have any commercial arrangement with its empanelled medical providers. The Medical Providers have agreed to offer their services at the Subsidised Fees / without any charge, as the case may be, for the cause of increasing health awareness, in particular preventive health care, and in no way shall it be construed that the empanelled Medical Providers are doing so for commercial reasons. Any Medical Provider proved to be offering the Medical Services / Additional Health Benefit Services for commercial reasons may be reported to IHO and, subject to furnishing of adequate proof, such Medical Provider shall be de-empanelled from IHO doctor’s network panel
    • IHO is a facilitator in providing access to quality healthcare services and in helping people save on their health care costs. IHO does not provide any Medical Services or Additional Health Benefit Services and is not responsible for the outcome of any services availed from any Medical Provider. All Medical Services and Additional Health Benefit Services are the responsibility of the concerned Medical Provider, to be carried out in consultation with you.

     

    • IHO offerings are not insurance and do not involve any reimbursement paid to you, or any other person holding a valid IHO’s Preventive Healthcare Plan Membership Card or any other membership card of IHO, for any of the services offered by IHO.
    • IHO reserves the right, in its sole discretion and without any obligation, to make improvements to, or correct any error or omissions in, any portion of the IHO’s Preventive Healthcare Plan Membership Card and/or any package issued to you by IHO and update information from time to time.

     

    • Except as otherwise specifically stated herein, IHO agrees to keep your information confidential as provided in our Privacy Policy. Any personal information, contact details, feedback data or similar information provided by you to IHO shall be treated as confidential information. You expressly acknowledge and agree that the calls made by you to IHO and/or the calls made to you by any Medical Provider shall be recorded for the purposes of quality and for maintaining the record of your health information, and may be shared with a Medical Provider, if required, from time to time.
    • You hereby agree that certain information in relation to the IHO’s Preventive Healthcare Plan any package issued by IHO, including in relation to any of the Medical Services and/or the Additional Health Benefit Services, whether provided by you or generated by IHO / any Medical Provider associated with IHO, may be shared amongst IHO and the Medical Providers strictly for the purposes of carrying out their respective obligations in relation to provision of the Medical Services and/or the Additional Health Benefit Services to you. In case you do not wish for IHO to use your information for such purpose, you shall be required to send a request regarding the same by way of email to service@ihomail.com. However, you expressly acknowledge and understand that doing so may affect the provision of Medical Services and/or the Additional Health Benefit Services to you in such manner as determined by IHO and/or a Medical Provider.

     

    • Subject to the provisions of applicable law, your usage of the IHO’s Preventive Healthcare Plan’s Membership and/or any package issued by IHO entitles IHO to contact you by way of telephone call, SMS or any other mode; provided, however, that if your phone number is registered on DND/DNC registry, communication by way of telephone call and/or SMS shall be in relation to the IHO’s Preventive Healthcare Plan and/or the package used by you and/or your membership with IHO.
    • Neither IHO nor any Medical Provider shall be obliged to provide any services otherwise than in compliance with applicable laws. IHO shall not be responsible for any claims arising out of technical failures which shall include failure in telephone lines, failure of servers, or failure to provide correct updates due to technical snags at any time. Further, IHO shall not be responsible for any claims arising out of any failure(s) which are beyond the control of IHO.

     

    • No Preventive Healthcare Plan Card or any package issued to you by IHO may be redeemed for cash or used to avail of any services / benefits not specified to be part of the IHO’s Preventive Healthcare Plan or the relevant package, as the case may be, at any time.
    • Any package offered by IHO may be issued in such manner and form, whether paper, electronic or otherwise, as it deems fit. If determined by IHO, such packages may be issued to / purchased by, and be redeemable by, members of IHO not holding IHO’s Preventive Healthcare Plan Membership Cards as well.

     

    • Any advice provided by any Medical Provider shall depend solely on the accuracy of the information provided by you. Neither IHO nor any Medical Provider shall be responsible for any damage, physical, psychological or financial, caused to you due to any improper/unintended use of the IHO’s Preventive Healthcare Plan, any package issued to you or any of the benefits available thereunder, which includes but is not limited to the interpretation of the advice given by any Medical Provider by you or by any unqualified personnel.

     

    • You agree to defend, indemnify and hold harmless IHO from and against any and all claims, damages, obligations, losses, liabilities, costs or debt, and expenses (including but not limited to attorney's fees) arising from: (i) any unauthorized or improper access and use of the IHO’s Preventive Healthcare Plan and/or any package issued to you by IHO; (ii) your violation of any term of these Terms; (iii) your violation of any law; or (iv) your violation of the rights of a third party, including the infringement by you of any intellectual property or other right of any person or entity. These obligations will survive any termination of the Terms.
    • You hereby agree that no Medical Provider providing any services over the telephone shall be obligated to provide call-back service in the event of a call-drop or any similar circumstance except as provided in these Terms.

     

    • In no event shall IHO and/or any Medical Provider be liable to you or any third party for any indirect, consequential, exemplary, incidental, special, or punitive damages, including lost profit damages arising from your use of the IHO’s Preventive Healthcare Plan Membership Card and/or any of the services offered to you by IHO in any manner. You shall not be entitled to specific performance of any of the Terms. The foregoing limitation of liability shall apply to the fullest extent permitted by law in the applicable jurisdiction and, regardless of the form of the action, will at all times be limited to the amount paid, if any, by you to IHO for the IHO’s Preventive Healthcare Plan. You specifically agree and understand that neither IHO nor any Medical Provider is liable to you for any content or illegal conduct of any third party and that you alone accept such risk of harm.
    • The views and suggestions expressed by Medical Providers in the course of providing the Medical Services and/or the Additional Health Benefit Services shall in no manner be construed as an endorsement by IHO of any such advice and views and IHO shall not be responsible in any manner for any such advice.

     

    • To the fullest extent permissible by law IHO disclaims all possible warranties or representations  about the accuracy or completeness of the content regarding the Medical Services and/or the Additional Health Benefit Services provided  by Medical Providers, nor can IHO be held liable or responsible for any (i) errors, omissions or inaccuracies of content or submissions; (ii) personal injury or property damage of any nature resulting from your access to and end use of the Medical Services and/or the Additional Health Benefit Services; (iii) any unauthorized access to or use of our secure servers and/or any and all personal information and/or financial information that may be contained therein; (iv) any deletion of data or interruption or discontinuance of transmission to or from or through the Medical Services and/or the Additional Health Benefit Services; and (v) any content or transactions that is contained or may be available outside the Medical Services and/or the Additional Health Benefit Services.
    • IHO shall not be responsible for any deficiency in service on part of any Medical Provider. Any complaint by you regarding the standard of service or quality, or anything in relation to the Medical Services and/or the Additional Health Benefit Services provided by any Medical Provider shall be solely handled and resolved directly by the relevant Medical Provider. IHO shall not be responsible for any such complaints.

     

    • Neither IHO nor any Medical Provider shall assume any liability or responsibility for any claims, damages, consequences and/or orders of any statutory bodies or consumer forums not arising from any default or negligence on the part of IHO or the relevant Medical Provider, as the case may be.
    • IHO is entitled, without any liability whatsoever, to refuse, limit, suspend, vary or discontinue the provision of any of the Medical Services and/or the Additional Health Benefit Services, and any matter relating thereto including but not limited to the manner and the form in which the Medical Services and/or the Additional Health Benefit Services may be made available to you, at any time for any reasonable cause, including but not limited to the following:

     

    • any violation of these Terms or of any applicable law, rule, regulation, order, direction, notification or the like issued by any competent authority;
    • technical failure, modification, up-gradation, variation, relocation, repair and/or maintenance of relevant computer or other systems/equipment;

     

    • any other reason found to be reasonable by IHO at its discretion.
    • Any provision of the terms and conditions that are declared invalid or unenforceable by a court of competent jurisdiction shall be ineffective to the extent of such invalidity or unenforceability without invalidating or rendering unenforceable the remaining provisions hereof.

     

    • IHO reserves the right to modify the Terms from time to time and it shall be your responsibility to keep yourself updated with the Terms. You agree to be bound by such modifications.
    • The provisions of the Terms are personal to you and you may not assign your rights and obligations under the Terms to any person.
    • You agree to comply with the local laws of the territory where you are located for usage of the IHO’s Preventive Healthcare Plan and/or any package issued to you by IHO, and IHO does not have any responsibility for any violation of laws by you. The Terms shall be governed by the laws of India and the courts at New Delhi will have exclusive jurisdiction thereupon.

      CANCELLATION POLICY

      CANCELLATION AND REFUND: In the event that you wish to cancel your membership with IHO and the IHO’s Preventive Healthcare Plan Membership Card issued to you, you shall be required to intimate the same to IHO by calling IHO’s help desk or by sending an email to service@ihomail.com within 14 (fourteen) days of receipt of the IHO’s Preventive Healthcare Plan Membership’s Welcome Pack. You shall further be required to return the IHO’s Preventive Healthcare Plan’s Membership’s Welcome Pack, with all documents and other items provided to you along with the Welcome Pack (“IHO’s Preventive Healthcare Plan Welcome Pack”) by mailing the aforesaid to the IHO Cancellation Department at 64, Okhla Industrial Estate, Phase – III, New Delhi – 110020. 

      On receipt of such intimation and the IHO’s Preventive Healthcare Plan’s Welcome Pack, IHO shall verify the details of your membership and the request for cancellation placed by you. On IHO’s satisfaction that your request is in accordance with these Terms, IHO shall carry out the following (collectively, “Cancellation”):

      • discontinue the IHO’s Preventive Healthcare Plan Membership Card issued to you and your membership with IHO with immediate effect; and

       

      • issue a refund of the amount paid by you as fees towards purchase of such IHO’s Preventive Healthcare Membership Plan  (“Membership Fees”).

      On Cancellation as aforesaid, IHO shall intimate you of the fact of such Cancellation and shall inform you of the method of refund of your Membership Fees. Such Cancellation shall be subject to the following terms:

      • Cancellations shall be considered only if (i) a valid request is made in this regard within 14 (fourteen) days of receipt of the IHO’s Preventive Healthcare Plan’s Welcome Pack by you, and (ii) the entire IHO’s Preventive Healthcare Plan’s Welcome Pack is returned to IHO within 14 (fourteen) days of receipt of the Cancellation request as aforesaid. For calculation of the aforesaid periods, the records of dispatch, delivery and communications available with IHO shall be considered final.

       

      • No Cancellation shall be carried out if you have purchased the IHO’s Preventive Healthcare Membership Plan under any celebratory / festive offer, or any special / discounted offer of any kind whatsoever.
      • No Cancellation shall be carried out if you have purchased the IHO’s Preventive Healthcare Membership Plan under any interest free easy monthly instalment (EMI) option offered by IHO.

       

      • No Cancellation shall be carried out if you have in any way availed any of the services/benefits available under the IHO’s Preventive Healthcare Plan’s Membership prior to confirmation of Cancellation by IHO.
      • In the event a request for Cancellation is received by IHO after dispatch of the IHO’s Preventive Healthcare Plan’s Welcome Pack to you, IHO shall deduct an amount of Rs. 1,000/- (Rupees one thousand only) or 10% (ten per cent) of the Membership Fees, whichever is higher, from your Membership Fees and only the balance shall be refunded to you as part of the Cancellation process.

       

      • In the event that (i) the entire IHO’s Preventive Healthcare Plan’s Welcome Pack is not received by IHO within 14 (fourteen) days of receipt of a valid request for Cancellation by IHO, or (ii) a request for Cancellation is received by IHO after a welcome call has been made by IHO to you wherein you have confirmed your membership, IHO shall be entitled to deduct an amount of Rs. 2,500/- (Rupees two thousand five hundred only) or up to 50% (fifty per cent) of your Membership Fees, whichever is higher, from your Membership Fees and only the balance shall be refunded to you as part of the Cancellation.
      • All Cancellations approved by IHO shall ordinarily be processed within 21 (twenty one) days of receipt by IHO of the IHO’s Preventive Healthcare Plan’s Welcome Pack. Credit of the Membership Fees to your bank account / credit card account upon Cancellation and refund by IHO shall be as per policies and timelines followed by the banks involved and IHO shall not be responsible for any delays in this regard.

       

      LOST CARD & COMPLAINTS

      LOST CARD: In case of loss of the IHO’s Preventive Healthcare Plan Membership Card, a duplicate membership card may be issued by IHO. You shall be required to pay nominal charges of Rs. 500/- (Rupees five hundred only) for issuance of such duplicate card. The tenure of your membership will remain as specified in the initial IHO’s Preventive Healthcare Plan Membership Card.

      COMPLAINTS: Members are advised to address their complaints, if any, to our representatives. They are there to help you out. However, if you are not satisfied, you may write to IHO at our corporate office address or mail us at service@ihomail.com.

      PROVISION OF SERVICES BY MEDICAL PROVIDERS: The provision of Medical Services provided by Medical Providers to you shall, in addition to as otherwise stated, be subject to the following:

      • The Medical Provider will offer the Subsidised Fees on Medical Services offered by the Medical Provider directly within the premises of such IHO’s medical network partner. The Subsidised rate on medical services shall not be applicable to any further service required from any other doctor/hospital, whether referred by the Medical Provider or otherwise. All medical services, tests, medical fields and specializations may not be under the IHO’s Preventive Healthcare Membership Plan. Further, the Subsidised Fees may not be offered on all treatments and procedures offered by the Medical Provider.

       

      • Any existing subsidised service or package offered or planned by a Medical Provider may not be clubbed with the Medical Services being offered at the Subsidised Fees, except at the option of the Medical Provider.
      • Before availing any Medical Service from any Medical Provider, you are advised to clarify in advance all rates and fees, including the Subsidised Fees, and all other details about the treatment, check-up, test, or other services offered by the Medical Provider to your satisfaction from the relevant Medical Provider. IHO will not participate in or be responsible for any dispute related to rates, Subsidised Fees, services offered or any other such detail once any Medical Service has been availed of.

       

      • If a particular Medical Provider does not offer the Subsidised Fees, you may contact IHO, and IHO will inform you of other Medical Providers partnered with IHO. IHO will not be responsible for providing any compensation to you for the charges levied by the Medical Provider in excess of the Subsidised Fees. 

       

       

      SYMPTOM TRACKER T&Cs

      SYMPTOM TRACKER: Specific Medical Providers may provide a recognition and advisory service on the basis of symptoms as specified by the user of such service (“Symptom Tracker”). The Symptom Tracker shall be open to all and shall not require payment of any consideration, unless otherwise specifically stated. The Symptom Tracker is a general, preliminary advisory service only. The Symptom Tracker is not intended to provide specific medical advice, but rather to provide users with information to better understand their health and apparent medical disorders with the appropriate medical advice. No specific medical advice will be provided via the Symptom Tracker, and users are urged to consult with medical experts for diagnosis and for answers to personal medical questions.TheSymptom Tracker is owned and operated by the relevant Medical Provider, and IHO is not in any way responsible for the layout, content or operation of, or the advice provided on the Symptom Tracker.

      ADDITIONAL BENEFITS: In addition to the concessional Medical Services offered, various additional benefits and plans are offered by Medical Providers to members of IHO (“Additional Health Benefit Services”). The features / benefits offered vary depending on the type of membership purchased by you. Certain Additional Health Benefit Services are available only to premier members of IHO, or are required to be purchased separately. Please refer to your Preventive Healthcare Plan Welcome Pack to check what benefits are offered to you. Certain terms and conditions are specific to certain membership plans and are stated below:

      DIAL –A-DOCTOR & 2ND OPINION T&Cs

      • IHO’s Dial – a – Doctor and Second Opinion: If applicable, you are entitled to the following services: (i) provision of interim medical advice over the phone (“IHO’s Dial-a-Doctor Service”); and (ii) provision of medical opinions over the phone regarding serious illnesses (“IHO’s Second Opinion Service”). The Dial-a-Doctor Service and the Second Opinion Service shall, in addition to as stated in these Terms, be subject to the following terms and conditions:

       

      • The Dial-a-Doctor Service and Second Opinion Service shall be provided by qualified doctors and medical practitioners on the rolls of Medical Providers (“Third Party Doctors”).
      • In order to avail of the Dial-a-Doctor Service or the Second Opinion Service, you shall be required to call the number intimated to you by IHO for this purpose. At the time of making such call, you shall be required to state your IHO membership number as provided on your IHO’s Preventive Healthcare Plan Membership Card and the provision of the aforesaid services to you shall be subject to such IHO membership number and your IHO’s Preventive Healthcare Membership Plan being found to be due and valid.

       

      • The Dial-a-Doctor Service and the Second Opinion Service will be available between 10 am - 8 pm on all working days, subject to availability of the concerned doctor. You may need to dial an outstation mobile phone number to access such services. All applicable call charges will be incurred by you.
      • You hereby confirm that you are aware that the Third Party Doctors receiving and attending calls from holders of the IHO’s Preventive Healthcare Plan’s Membership shall have a minimum qualification of Bachelor of Science (BS).

       

      • Any advice provided by any Third Party Doctor shall not be construed as final and shall be subject to a physical check-up conducted by a duly qualified medical professional. In case of any requirement you may have which cannot, in the Third Party Doctor’s reasonable opinion, be addressed over the telephone, you are advised to physically consult the required medical specialist.
      • You hereby acknowledge that the Third Party Doctors providing the Dial-a-Doctor Service shall provide interim medical advice only as such Third Party Doctor cannot check you physically. No sensitive or critical advice or prescription of any kind shall be given to you by any Third Party Doctor providing the Dial-a-Doctor Service.

       

      • You hereby further acknowledge that the Second Opinion Service shall be provided to you on the basis of medical tests undergone by you and opinions taken by you from duly qualified medical professional prior to availing the Second Opinion Service. Any advice provided to you by a Third Party Doctor as part of the Second Opinion Service shall be dependent on such preliminary tests / consultancies.

      DOCTOR CHAT T&Cs

      • IHO’s Chat-with-Doctor Service: If applicable, you are further entitled to the online interim medical advice facility provided Medical Providers to members of IHO (“Chat-with-Doctor Service”). The Chat-with-Doctor Service shall, in addition to as stated in these Terms, be subject to the following terms and conditions:

       

      • The Chat-with-Doctor Service shall also be provided by Third Party Doctors.
      • The Chat-with-Doctor Service may be availed by IHO members as well as non-members in accordance with these Terms. Only general physician consultations will be provided as part of the Chat-with-Doctor Service.

       

      • In order to avail of the Chat-with-Doctor Service, you shall be required to register on the website of IHO (www.indianhealthorganisation.com/login). You shall not be required to pay any amount for such registration. After registration, you may start using the Chat-with-Doctor Service.
      • You shall be entitled to ask general queries by using the Chat-with-Doctor Service, which shall be answered by the Third Party Doctors in real time to the best of their abilities. The Chat-with-Doctor Service is intended to provide you with the following benefits:

       

      •  
        • General advice on your health condition / health issues, probable diagnosis, prognosis etc., as well as on the broad management of your health conditions.
      •  
        • Information on over the counter medication for temporary relief.

       

      •  
        • Doubt clarification on any investigation you have gone through.
      •  
        • Advice on diet for conditions like diabetes, hypertension, weight loss, weight gain etc.

       

      •  
        • Advice on kind of doctors you need to consult for your conditions.
      • The Chat-with-Doctor Service will be available between 8 am - 7 pm on all days (except national holidays), subject to these Terms.

       

      •  In the event you log onto the website of IHO and submit a query at any time other than as specified above, the  service gets converted into a question – answer format whereby the user can send your query in the form of a question on the given e mail ID visible on the chat pop up box , you will get a response within 24 hours via e-mail, else you can log on the next day during live hours
      • For IHO members post 7 pm, the dial a doctor facility is available, where you can have a telephonic chat with the doctors.

       

      • Any advice provided by any Third Party Doctor shall not be construed as final and shall be subject to a physical check-up conducted by a duly qualified medical professional. In case of any requirement you may have which cannot, in the Third Party Doctor’s reasonable opinion, be addressed online, you are advised to physically consult the required medical specialist.
      • You hereby acknowledge that the Third Party Doctors providing the Chat-with-Doctor Service shall provide interim medical advice only as such Third Party Doctor cannot check you physically. No sensitive or critical advice or prescription of any kind shall be given to you by any Third Party Doctor providing the Chat-with-Doctor Service.

       

      • You hereby further acknowledge that you shall be entitled to receive advice on over the counter medications only. The Third Party Doctor shall only prescribe medication which is generally considered to be safe under all circumstances. In case of any allergies, you are advised to undergo personal physical consultation with a nearby doctor.
      Any queries by you in relation to the Chat-with-Doctor Service should be intimated to IHO by sending an email to IHO at doctorchat@ihomail.com, or by calling IHO at 011-60004466 between 9 am and 6 pm (Monday to Saturday). IHO will endeavour to address all queries within 48 (forty eight) working hours.

      IHO- THYROCARE’S T&Cs

       

      1. Thyrocare Health Check Packages: If applicable, you are further entitled to certain packages of diagnostic and investigative tests and services offered by IHO through Thyrocare Technologies Limited (“Thyrocare”) for free / at discounted rates. Such packages may be availed by using either the classic package (“IHO-Thyrocare’s Classic Package”) or the premier package (“IHO- Thyrocare’s Premier Package”) issued by IHO. The Classic IHO-Thyrocare Package entitles you to avail the IHO-Thyrocare Package A and the IHO-Thyrocare Package B, and the IHO-Thyrocare Premier Package entitles you to avail the IHO-Thyrocare Premier Package. “IHO-Thyrocare Package A” means the package of diagnostic and investigative tests and services designated as such, the detailed list of which is located at [http://wellness.thyrocare.com/public_tests_list1.asp]. “IHO-Thyrocare Package B” means the additional package of diagnostic and investigative tests and services designated as such, the detailed list of which is located at [http://wellness.thyrocare.com/public_tests_list1.asp]. “IHO- Thyrocare Premier Package” means the package of diagnostic and investigative tests and services designated as such offered to holders of IHO’s Preventive Healthcare Membership Plans, the detailed list of which is located at [http://www.thyrocare.com/iho.htm]. The aforesaid packages shall, in addition to as stated in these Terms, be subject to the following terms and conditions 
      • The Classic IHO–Thyrocare’s Package and the Premier IHO-Thyrocare’s Package (collectively, the “Thyrocare Packages”) shall be issued by IHO in manner as may be determined by IHO at its sole discretion.

       

      • You may redeem the Classic IHO –Thyrocare’ s Package only once in order to avail the IHO- Thyrocare’s Package A either by itself or along with the IHO –Thyrocare’s Package B. It is hereby clarified that you may avail of IHO-Thyrocare’s Package A as well as IHO- Thyrocare’s Package B only if you avail of both simultaneously. You shall not be entitled to avail the IHO –Thyrocare’s Package’s B without availing the IHO –Thyrocare’s Package A.
      • You may redeem the IHO-Thyrocare’s Premier Package only once in order to avail the IHO   -Thyrocare’s Premier Packages. It is hereby clarified that you may redeem the IHO- Thyrocare’s Premier Package and the IHO- Thyrocare’s Classic Package either separately or simultaneously with each other.

       

      • You may avail the IHO –Thyrocare’s Package A and/or the IHO- Thyrocare’s Premier Package free of any cost, including collection of sample(s) and delivery of reports at the address prescribed by you. You may avail the IHO –Thyrocare’s Package B by paying an amount of Rs. 200 (Rupees two hundred only) directly to Thyrocare technician at the time of collection of your blood sample in case home collection services are availed, or when you visit Thyrocare collection centre.
      • Each Thyrocare Package shall have a unique identification number (“Thyrocare Package Number”) and date of expiry (“Thyrocare Package Expiry Date”) printed/stamped on your package coupon. You may use the relevant Thyrocare Package, subject to the provisions, T & C’s stated on your coupon, at any time up till the Expiry Date.

       

      Usage process for the IHO-Thyrocare’s Health Check Packages

         (a). All IHO –Thyrocare’s (Classic & Premier) package issued till 30th April can be used by  calling Thyrocare to book an appointment to Thyrocare customer care number (022 - 30900000 / 41252525 / 27622762) or by sending an  SMS “IHOP” to 9870666333 or by sending an email to info@thyrocare.com.

      • In case you are unable to communicate with Thyrocare in relation to redemption of a Thyrocare Package through the modes stated herein, you may send an email regarding the same to [service@ihomail.com].

       

      (b). All IHO- Thyrocare’s packages  issued from 1st July 2013 onwards, you shall be required to call IHO Helpdesk for booking of appointment/usage at below numbers:

      (c). At the time of making such booking, you shall be required to state (i) your IHO-Thyrocare’s Package Number and the IHO-Thyrocare’s Package Expiry Date thereof; (ii) if redeeming the Classic IHO-Thyrocare’s  Package, whether you wish to avail of IHO- Thyrocare’s Package A by itself or along with the IHO –Thyrocare’s Package B; (iii) address for collection of sample(s) and delivery of report; (iv) your contact number, preferably your mobile phone number; and (v) 2 (two) alternative convenient dates and timings for collection of sample(s) by authorised technician sent by Thyrocare. The details of the booking placed with IHO/ Thyrocare shall be sent to you by way of an email and an SMS. You may avail the IHO –Thyrocare’s Premier Package, the IHO-Thyrocare’s Package A and/or the IHO- Thyrocare’s Package B by handing over a copy of your valid & booked IHO-Thyrocare‘s Package.

       

      • You shall be required to undergo fasting for a minimum of 12 (twelve) hours prior to providing sample(s) for any of the tests specified. It is recommended that you undergo overnight fasting prior to provision of sample(s). Additional directions may be provided to you by Thyrocare at the time of placing the booking, if required. IHO shall not be responsible in any manner whatsoever if you do not undergo fasting as prescribed herein or follow any directions provided to you.
      • You shall be required to obtain confirmation directly from Thyrocare regarding availability of authorised Thyrocare service centre and/or provision for collection of samples and delivery of reports at address prescribed by you (“Home Service”) in your city. In the event such Home Service is not available in your city, you shall be required to visit the nearest Thyrocare service centre for providing sample(s) and collecting reports. The list of cities where Thyrocare service centre is located is subject to change at any time without prior notice.

       

      • Reports for the tests carried out shall be made available within 3 (three) to 5 (five) days after collection of sample(s) at (i) the address specified by you, if Home Service is available at your location; or (ii) the Thyrocare service centre where you provided the sample(s). Reports may also be provided to you by email if requested.
      • In the event of cancellation of tests by Thyrocare for various clinical reasons such as the sample becoming unfit for processing due to inadvertent reasons like hemolysation or clotting or quality deterioration of sample, sample becoming insufficient due to multiple retesting of certain parameters, absurd values generated due to reasons not under control etc., Thyrocare may re-conduct the test/investigation desired by you. Such retesting, including collection of sample(s) from and delivery of reports to the address specified by you, shall be conducted free of any costs or charges of any nature whatsoever to you. However if the reports for the test/investigation desired by you are released and you thereafter ask for retesting, you shall be charged for such retesting as per the prevailing Thyrocare rates.

       

      • Neither IHO nor Thyrocare shall be responsible for any delays in getting the sample(s) to the central processing laboratory of Thyrocare or in delivery of reports which may arise due to causes beyond their organisational control such as delay by courier agency, unexpected cancellation of commercial flights due to extreme weather conditions, natural calamities or any other force majeure condition, or your absence or unavailability at the address specified by you when report delivery is attempted. In such an event, Thyrocare may, at its discretion and on a case to case basis, re-conduct the test/investigation desired by you, including collection of sample(s) from and delivery of reports to the address specified by the you, free of any costs or charges of any nature whatsoever to you.
      • The IHO-Thyrocare’s Premier Package, the IHO-Thyrocare’s Package A and/or the IHO –Thyrocare’s Package B may be withdrawn at any time including during the validity period of the relevant Thyrocare’s Package at the sole discretion of IHO with or without prior notice.

       

      • Any results generated from any tests conducted through usage of any Thyrocare Package may not be used for any medico-legal purposes.
      • All members test reports will be shared by Thyrocare with IHO and IHO may use this information, in case an IHO member would not like IHO to use your test reports, he/she must send the mail request (at least 3 days before tests to service@ihomail.com)

       

      • Thyrocare shall be solely responsible for any defect or deficiency with regard to conducting tests/investigations, collection of sample(s) and preparation and delivery of reports, including without limitation deficiency in quality of testing, default in meeting timelines or otherwise. IHO shall not be responsible for any deficiency in service on part of Thyrocare. Any complaint by you regarding the standards of service or quality, or anything in relation to the services provided by Thyrocare, shall be solely handled and resolved directly by Thyrocare. IHO shall not be responsible for any such complaint.
      • Neither IHO nor Thyrocare shall be responsible for any issues arising in case you directly avail service from a service provider without making a prior booking with Thyrocare as per the procedure stated herein.

       

      • All IHO-Thyrocare’s packages are valid till the IHO membership is valid.

      DIET FIT PACKAGE T&Cs

      • Diet Fit Package: If applicable, you are further entitled to customized diet packages designed and provided by Medical Providers to members of IHO (“Diet Fit Packages”). Such diet packages may be availed by using the specialised package issued by IHO for this purpose (“Diet Fit Package”). The Diet Fit Packages shall, in addition to as stated in these Terms, be subject to the following terms and conditions: 

       

      • The Diet Fit Package shall be issued by IHO to you in such manner as may be determined by IHO at its sole discretion. The IHO’s Diet Fit Packages may be redeemed only by holders of a valid IHO’s Diet Fit Package. The IHO’s Diet Fit Package shall have a unique identification number (“IHO’s Diet Fit Package Number”) and date of expiry (“IHO’s Diet FitPackage Expiry Date”) provided on it. You may use the IHO’s Diet Fit Package to avail of the relevant IHO’s Diet Fit Package, subject to the provisions stated in these Terms, at any time up till the IHO’s Diet Fit Package Expiry Date.
      • You shall be entitled to receive the following as part of every IHO’s Diet Fit Package:

        • Phone call from an experienced specialist, including a duly qualified doctor and nutritionist, on the roster of a Medical Provider (“Dietician”) to understand your overall lifestyle pattern and to provide customised diet plans to you based on the information provided by you (“IHO’s Diet Fit Consultation Call”);
      •  
        • Phone call from the Dietician enquiring about how and what has been your development so far (“Follow-up Call”);

       

      •  
        • Daily SMS consisting of dietary health tips customised as per your medical health requirements (“Diet Fit SMS”); and
      •  
        • Email newsletter consisting of diet plans, recipes and diet charts customised as per your requirements along with general dietary health tips (“Diet Fit Email”).

       

      • The IHO’s Diet Fit Package purchased by you may be of a duration of (a) 1 (one) month (“One Month Diet Fit Package”); (b) 3 (three) month (“Three Month Diet Fit Package”); or (c) 6 (six) months (“ Six Month Diet Fit Package”). You shall be entitled to the following based on the Diet Fit Package purchased by you:
      • The One Month’s IHO’s Diet Fit Package shall consist of:
      • 1 (one) Diet Fit Consultation Call;
    • 1 (one) Follow-up Call;
    • 30 (thirty) Diet Fit SMSs; and
    • 4 (four) Diet Fit Emails.
    • The Three Months IHO’s Diet Fit Package shall consist of:
    • 1 (one) Diet Fit Consultation Calls;
  • 2 (two) Follow-up Calls;
    • 90 (ninety) Diet Fit SMSs; and
    • 12 (twelve) Diet Fit Emails.
    • The Six Months IHO’s Diet Fit Package shall consist of:
    • 1 (one) Diet Fit Consultation Calls;
    • 5 (five) Follow-up Calls;
    • 180 (one hundred and eighty) Diet Fit SMSs; and
    • 24 (twenty four) Diet Fit Emails.
    • To avail of the relevant IHO’s Diet Fit Package, you shall be required to contact IHO by calling the relevant help-desk number below between 9 am and 6 pm (Monday to Saturday) (“Diet Fit Registration Call”):

    USAGE PROCESS / VALIDITY:

    • You shall be required to make the IHO’s Diet Fit Registration Call as aforesaid within the following periods:

     

    •  
      • If you have purchased the One Month IHO’s Diet Fit Package, within 45 (forty five) days of the date of purchase of the IHO’s Diet Fit Package;
    •  
      • If you have purchased the Three Months IHO’s Diet Fit Package, within 90 (ninety) days of the date of purchase of the IHO’s Diet Fit Package; and

     

    •  
      • If you have purchased the Six Months Diet Fit Package, within 90 (ninety ) days of the date of purchase of the IHO’s Diet Fit Package.
    • At the time of making the IHO’s Diet Fit Package’s Registration Call to the IHO helpdesk, you shall be required to state your IHO’s Diet Fit Package Number as provided on your IHO’s Diet Fit Package and the provision of benefits under the IHO’s Diet Fit Packages to you shall be subject to such IHO’s Diet Fit Package Number being found to be due and valid. You shall also be required to provide certain additional details to the IHO help desk executive during the IHO’s Diet Fit Registration Call, including the following:

     

    •  
      • Full demographic details as requested;
    •  
      • Contact details (valid mobile number and email ID);

     

    •  
      • Vital statistics, i.e., height in feet, weight in kilograms, waist circumferences in centimetres, etc;
    •  
      • IHO’s Diet Fit Package’s duration;

     

    •  
      • Purchase date of the relevant IHO’s Diet Fit Package;
    •  
      • IHO’s Diet Fit Package’s Expiry Date.

     

    • In addition to the above, the IHO help desk representative shall fill out a questionnaire about your lifestyle pattern and dietary habits (“Questionnaire”) on your behalf during the IHO’s Diet Fit Registration Call. For such purpose, you shall be required to provide all information as may be requested by the IHO help desk representative.
    • You shall further be required to provide to the IHO help desk representative during the IHO’s Diet Fit Registration Call, to be included in the Questionnaire, two alternative convenient dates and times on which the Dietician may make the Consultation Call. Such convenient dates and times should be not less than 48 (forty eight) hours and not more than 7 (seven) days from the time of the Registration Call.

     

    • Within 24 (twenty four) hours of successful completion of the IHO’s Diet Fit Package’s Registration Call, you shall be sent an e-mail and/or an SMS confirming submission of the Questionnaire and stating the date and time, from amongst the two alternative dates and times specified by you, on which the Consultation Call shall occur. The email and/or SMS sent to you shall also contain a link to a website along with login details and password which you may use to access your customised IHO’s Diet Fit Package portal page. You shall be required to access such customised IHO’s Diet Fit Package portal page using the login details and password provided within 7 (seven) days of receiving the same, post which the link and login details as provided shall be deactivated.
    • You may modify the preferred dates and times for the Consultation Call provided by you during the IHO’s Diet Fit Package’s Registration Call through your IHO’s Diet Fit Package’s portal page at any time prior to 24 (twenty four) hours before your scheduled Consultation Call, provided that such rescheduled date and time should not be less than 48 (forty eight) hours from the time of submission of your amended date and time.

     

    • On the date and time stated, the Dietician shall make the Consultation Call. Such Consultation Call shall not be made unless the Questionnaire is duly filled up and submitted as required. The Consultation Call may last up to approximately 30 (thirty) minutes and you should make himself available accordingly. In the event you are not available at the specified time, the Dietician shall call later on the same date or, failing that, on the immediately succeeding day.
    • Provision of the IHO’s Diet Fit Package SMSs to you shall commence within 24 (twenty four) hours of the Consultation Call. Further, provision of the IHO’s Diet Fit Package Emails to you shall commence within 48 (forty eight) hours of the Consultation Call.

     

    • You shall receive the first Follow-up Call within 15 (fifteen) days of the Consultation Call. Any further Follow-up Calls to be made based on the Diet Fit Package purchased by you shall be made not later than 30 (thirty) days after the immediately preceding Follow-up Call. In the event the Dietician is unable to reach you for any Follow-up Call, he/she shall make the relevant Follow-up Call later during the same day or on the immediately succeeding day.
    • The IHO’s Diet Fit Package SMSs shall be sent to you once a day during the tenure of your Diet Fit Package on the mobile number provided by you to IHO help desk representative during the Registration Call. Further, the IHO’s Diet Fit Package Emails shall be sent to you once a week during the tenure of your IHO’s Diet Fit Package on the email address provided by you to IHO. The number of IHO’s Diet Fit Package SMSs and IHO’s Diet Fit Package Emails shall however be subject to the maximum prescribed hereinabove for the relevant IHO’s Diet Fit Package purchased by you.

     

    • In the event you wish to extend the tenure of your IHO’s Diet Fit Package, you shall be required to purchase a new IHO’s Diet Fit Package by contacting IHO by calling the relevant help-desk number provided hereinabove between 9 am and 6 pm (Monday to Saturday) (“Diet Fit Renewal Call”). During the IHO’s Diet Fit Package’s Renewal Call, you shall be required to make payment for the relevant new IHO’s Diet Fit Package you wish to purchase. Further, at the time of making the IHO’s Diet Fit Package’s Renewal Call, you shall be required to provide certain details to the IHO help desk representative as requested during the IHO’s Diet Fit Package’s Renewal Call, as well as provide all requisite details to the IHO help desk representative in order to enable the IHO help desk representative to complete a short questionnaire (“Package Renewal Questionnaire”).
    • All provisions of these Terms in relation to the IHO’s Diet Fit Package’s Registration Call shall apply mutatis mutandis to the Diet Fit Package’s Renewal Call, and all provisions of these Terms in relation to the Questionnaire shall apply mutatis mutandis to the Renewal Questionnaire. Further, the procedure as described in these Terms after successful completion of the IHO’s Diet Fit Registration Call shall be mutatis mutandis applicable after successful completion of the IHO’s Diet Fit Package’s Renewal Call.

     

    • The truncated procedure relating to IHO’s Diet Fit Package’s Renewal Call and Renewal Questionnaire as specified hereinabove shall be applicable only if (a) the Diet Fit Renewal Call is made by you to IHO by calling the relevant help-desk number as provided above within 1 (one) year of expiry of your previous Diet Fit Package; and (b) the user of the initial IHO’s Diet Fit Package remains the user of the subsequently purchased Diet Fit Package. In case you wish to purchase a new IHO’s Diet Fit Package after the expiry of the aforesaid period of 1 (one) year, or if the user of the new IHO’s Diet Fit Package has changed, you shall be required to purchase an IHO’s Diet Fit Package and comply with the provisions relating to IHO’s Diet Fit Package’s Registration Call and Questionnaire as specified hereinabove.
    • You hereby acknowledge and agree that you shall not be entitled to any of the benefits arising from the relevant IHO’s Diet Fit Package purchased by you in the event any of the actions required to be taken by you as stated in these Terms are not duly taken by you within the relevant time periods specified herein.

     

    • You hereby acknowledge that the Dieticians shall provide interim medical advice only as such Dietician cannot check you physically. Any advice provided by any Dietician shall not be construed as final and shall be subject to a physical check-up conducted by a duly qualified medical professional. No sensitive or critical advice or prescription of any kind shall be given to you by any Dietician. In case of any requirement you may have which cannot, in the Dietician’s reasonable opinion, be addressed over the telephone, you are advised to physically consult the required medical specialist.
    • Any queries by you in relation to the IHO’s Diet Fit Package should be intimated to IHO by sending an email to IHO at diet.q@ihomail.com or by calling the relevant help-desk number provided hereinabove between 9 am and 6 pm (Monday to Saturday). IHO will endeavour to address all queries within 48 (forty eight) working hours.

     

    PRE-NATAL CARE PACKAGE’S T&Cs

    • Pre-natal Care Package If applicable, you are further entitled to pre-natal care related health information provided by Medical Providers to members of IHO (“Pre-natal Care Package”). Such programs may be availed by using the specialised package issued for this purpose (“Pre-natal Care Program Package”). The Pre-natal Care Package shall, in addition to as stated in these Terms, be subject to the following terms and conditions: 

     

    • The Pre-natal Care Program Package shall be issued by IHO to you in such manner as may be determined by IHO at its sole discretion. Pre-natal Care Programs may be availed of only by holders of a valid Pregnancy Plan Package. The Pre-natal Care Program Package shall have a unique identification number (“Pre-natal Care ProgramNumber”) and date of expiry (“Pre-natal Care Package’s Expiry Date”) provided on it. In the event you hold a membership with IHO, the Pre-natal Care Package’s Expiry Date shall be the earlier of (i) the date of expiry printed on the Pre-natal Care Package, or (ii) the last date of validity of your IHO membership. You may use the Pre-natal Care Package to avail of the Pre-natal Care Package, subject to the provisions stated in these Terms, at any time up till the Pre-natal Care Package’s Expiry Date.
    • To avail of the Pre-natal Care Package, you shall be required to register your Pre-natal Care Package (“ Pre-natal Care Package’sRegistration”) by either (i) visiting the registration page provided for this purpose at [insert registration link]; or (ii) contacting IHO by calling the relevant help-desk number below between 9 am and 6 pm (Monday to Saturday):

    • You shall be required to complete the Pre-natal Care Package’s Registration as aforesaid, prior to the Pre-natal Care Package’s Expiry Date, between the date of conception and the pregnancy due date.

     

    • At the time of Pre-natal Care Package’s Registration, you shall be required to state your Pre-natal Care Package Number as provided on your Pre-natal Care Package and the provision of benefits under the Pre-natal Care Package to you shall be subject to such Pre-natal Care Package Number being found to be due and valid. You shall also be required to provide certain additional details as part of the Pre-natal Care Package’s Registration, including the following:
    •  
      • Full demographic details as requested;

     

    •  
      • Contact details (valid mobile number and email ID);
    •  
      • Delivery due date;

     

    •  
      • First day of last menstrual period;
    •  
      • Number of days of pregnancy;

     

    •  
      • Pre-natal Care Package’s Expiry Date.
    • Within 24 (twenty four) hours of successful completion of the Pre-natal Care Package’s Registration, you shall be sent an e-mail and/or an SMS containing your login details and password for accessing your customised portal (“Customer Portal”) along with link for your customised portal. You may visit your Customer Portal using the login details and password provided at any time during the validity period of your Pre-natal Care Package.

     

    • Upon completion of the Pre-natal Care Package’s Registration, you shall be entitled to receive the following:
    •  
      • Phone consultations with various medical specialists on the roster of a Medical Provider (“Specialists”) in accordance with your needs (“Pre-natal Care Package’s ConsultationCall”);

     

    •  
      • SMS consisting of estimated changes occurred / occurring in the baby based on estimated stage of pregnancy calendar (“Pre-natal Care Package SMS”);
    •  
      • Email consisting of estimated changes occurred / occurring in the mother and the baby (“Pre-natal Care Package Email”); and

     

    •  
      • Email newsletter aimed at education and information, containing tips on nutrition, pre-natal care, yoga and relaxation techniques, balancing work pressure and pre-natal care, diet, emotional health etc. (“Pre-natal Care Package Newsletter”).
    • All benefits provided to you as part of the Pre-natal Care Package shall be provided to you commencing from the successful completion of the Pre-natal Care Package’s Registration till the earlier of (i) your delivery due date as specified by you; or (ii) the expiry of a period of 9 (nine) months from the Pre-natal Care Package’s Registration.

     

    • Provision of Pre-natal Care Package SMSs to you shall commence within 24 (twenty four) hours of the Pre-natal Care Package’s Registration. Further, provision of Pre-natal Care Program Emails and Pre-natal Care Package Newsletters to you shall commence within 48 (forty eight) hours of the Pre-natal Care Package’s Registration.
    • The Pre-natal Care Package’s SMSs shall be sent to you once a day during the tenure of your Pre-natal Care Package. The Pre-natal Care Package’s Emails shall be sent to you once a week during the tenure of your Pre-natal Care Package. The Pre-natal Care Package Newsletters shall be sent to you once a month during the tenure of your Pre-natal Care Package. All of the aforesaid shall be sent on the mobile phone number or email, as the case may be, provided by you during the Pre-natal Care Package’s Registration.

     

    • You shall be entitled to have Pre-natal Care Package Consultation Calls with the following Specialists: (i) general physician, (ii) dietician, (iii) gynaecologist, or (iv) psychologist.
    • In order to have a Pre-natal Care Package Consultation Call, you shall be required to call IHO on 011-60004466 between 9 am and 6 pm (Monday to Saturday). In case you wish to consult with a general physician or a dietician, you may either (i) book a future date and time within the operating hours as specified below for such call, or (ii) have the Pre-natal Care Package Consultation Call immediately if within the operating hours specified below, in which case the IHO help-desk representative shall connect your call to the relevant Specialist. In case you wish to consult with a gynaecologist or a psychologist, you shall be required to book a future date and time within operating hours to have the Pre-natal Care Package Consultation Call. Each time you wish to have / book a Pre-natal Care Package Consultation Call, you may choose a particular Specialist to consult by checking the credentials of each Specialist on your Customer Portal.

     

    • Any Pre-natal Care Package Consultation Call booked by you shall be required to be within the operating hours of 9 am and 6 pm (Monday to Friday) and shall be not less than 2 (hours) after the booking call made by you.
    • You may modify the date and time of the booked Pre-natal Care Package Consultation Cal by calling the relevant IHO help-desk number as stated hereinabove, provided that such rescheduled date and time should not be less than 2 (two) hours from the time of amended booking made by you.

     

    • On the date and time stated, the relevant Specialist shall make the Pre-natal Care Program Consultation Call by calling you on the phone number proved by you during Pre-natal Care Program Registration. The Pre-natal Care Package Consultation Call should be specifically related to your pregnancy and you should make yourself available accordingly. In the event you are not available at the specified time or within a reasonable amount of time afterwards, the Specialist shall not be responsible for calling you again, and you shall be required to re-book a Pre-natal Care Package’s Consultation Call.
    • Even though this package is valid for 9 months, this package will expire at the time of delivery irrespective of date of registration or usage days of this package. IHO’s network concession benefits under this package will only be available till the validity of your IHO’s membership card.

     

    • After successful completion of any Pre-natal Care Package’s Consultation Call, you shall receive an email from the relevant Specialist containing feedback from such Pre-natal Care Package’s Consultation Call.
    • You shall also be entitled to use your Customer Portal for raising queries related to your pregnancy, which shall be answered by the relevant Specialists to the best of their abilities. In addition, you may also upload your pre-natal and related medical records on your Customer Portal for storage as well as review by the Specialists. Further, in the event you undergo testing at any diagnostic centre associated with IHO wherein such service is provided, you shall have the option of requiring such diagnostic centre to directly upload your test results on your Customer Portal. All records uploaded to your Customer Portal shall be maintained and shall be available till the validity of your Pre-natal Care Package, i.e., till the earlier of (i) your delivery due date as specified by you; or (ii) the expiry of a period of 9 (nine) months from the Pre-natal Care Package’s Registration. You shall not be able to access any of your records after the validity as aforesaid is over. Further, all such uploaded records shall be visible to all specialists as well as to IHO, and you agree to allow IHO and the specialists to view such records at any time during the validity of your Pre-natal Care Package.

     

    • You hereby acknowledge and agree that you shall not be entitled to any of the benefits arising from the relevant Pre-natal Care Package purchased by you in the event any of the actions required to be taken by you as stated in these Terms are not duly taken by you within the relevant time periods specified herein.
    • You hereby acknowledge that any information provided to you through any Pre-natal Care Package’s SMS, Pre-natal Care Package’s Email, and Pre-natal Care Package’s Newsletter or otherwise shall be based on the information provided by you as well as certain estimates made by the Specialists on the basis of the information provided by you, including estimates of your pregnancy cycle. As such, all such information shall be tentative and shall be subject to verification by way of physical check-up conducted by a duly qualified medical professional.

     

    • In the event you hold an IHO membership, you shall also be entitled to all benefits available to you under your IHO membership, including benefits on gynaecology consultations, laboratory investigations (pathology and radiology) and delivery concession at IHO empanelled medical centres and hospitals. For further details on network benefits, you may contact IHO help-desk at 011-60004466.
    • You hereby acknowledge that the Specialists shall provide interim medical advice only as such Specialists cannot check you physically. Any advice provided by any Specialist shall not be construed as final and shall be subject to a physical check-up conducted by a duly qualified medical professional. No sensitive or critical advice or prescription of any kind shall be given to you by any Specialist. In case of any requirement you may have which cannot, in the Specialist’s reasonable opinion, be addressed over the telephone, you are advised to physically consult the required medical professional.

     

    • Any queries by you in relation to the Pre-natal Care’s Package or the Pre-natal Care Package should be intimated to IHO by sending an email to IHO at maternity.q@ihomail.com, or by calling the relevant help-desk number provided hereinabove between 9 am and 6 pm (Monday to Saturday). IHO will endeavour to address all queries within 48 (forty eight) working hours.

    DENTAL PACKAGES T&Cs

    • Dental Package: If applicable, you are further entitled to certain dental scaling and cleaning services provided by dental service providers to members of IHO (“Dental Package”). Such package may be availed by using the specialised package issued by IHO for this purpose (“Dental Package”). The Dental Package shall, in addition to as stated in these Terms, be subject to the following terms and conditions: 

     

    • The Dental Package shall be issued by IHO to you in such manner as may be determined by IHO at its sole discretion. The Dental Package may be redeemed only by holders of a valid Dental Package. The Dental Package shall have a unique identification number (“Dental Package Number”) and date of expiry (“Dental Package Expiry Date”) provided on it. You may use the Dental Package to avail of the Dental Package, subject to the provisions stated in these Terms, at any time up till the Dental Package Expiry Date.
    • As part of the Dental Package, you shall be entitled to one session of basic oral prophylaxis treatment consisting of ultrasound cleaning and scaling of teeth free of cost. The Dental Package does not include any advanced cleaning processes like ozone based cleaning or any other process or benefit whatsoever.

     

    • To use the Dental Package, you shall be required to contact IHO by calling IHO at 011-60004466 between 9 am and 6 pm (Monday to Saturday) (“Booking Call”):
    • At the time of making the Booking Call, you shall be required to state your Dental Package Number and Dental Package Expiry Date thereof, and the provision of benefits under the Dental Package to you shall be subject to such Dental Package being found to be due and valid.

     

    • On receiving a Booking Call, the representative of IHO shall inform you of the Medical Providers with whom the Dental Package may be availed situated nearest to your location as intimated to IHO. The final selection of any of the alternatives provided shall be at your sole discretion.
    • You shall be required to provide 2 (two) alternative dates and times for making a booking for availing the benefits under the Dental Package to the IHO representative during the Booking Call. In the event a booking is unable to be made during either of the 2 (two) alternative dates and times provided by you, a representative of IHO shall call you on the number provided by you during the Booking Call with suggested alternative dates and times for such booking. You shall be required to confirm the booking at such date and time.

     

    • When a booking is successfully made for either of the alternatives as specified by you, or for the suggested date and time confirmed by you subsequently, IHO shall send confirmation of such booking to you by way of SMS to the number provided by you during the Booking Call.
    • You shall be required to present a copy of your Dental Package at the time of availing the Dental Package.
    • In the event you wish to change your booking date and time, you shall be entitled to do so by calling the relevant help-desk number during the times mentioned hereinabove not less than 24 (twenty four) hours prior to your scheduled booking. Any re-booking shall be subject to availability of the Medical Provider. In the event you fail to call the relevant IHO help-desk number within the time period as aforesaid, you shall not be entitled to any re-booking or further opportunity to avail the benefits of the Dental Package.
